Job Type: Permanent / Direct Hire
Setting: Hospice
Location: Elm Grove, WI
Schedule: Full Time, typically M – F 8:30am - 5pm, occasional on-call
Pay: Pay between $36/hr-$40/hr depending on years of experience
The Registered Nurse Case Manager duties to include but not limited to:
- Plan, organize, and direct hospice care utilizing the nursing process of assessment planning, interventions, implementation and evaluation; and effectively interacts with patients, caregivers, families and other interdisciplinary team members
- Collaborate with an Interdisciplinary Team regarding the eligibility and appropriateness of the client for hospice services
- Apply nursing principles and techniques in the treatment of patients
- Encourages patients and family members to participate in the patient's plan of care to meet established goals
