Overview
The Enrollment Coordinator is the first face of LifeCircles when they meet with potential PACE members and their families in their homes. The Enrollment Coordinator conducts clinical assessments, collaborates with the interdisciplinary care team, and with external professionals to help potential members enroll in PACE or find a solution that meets their needs. The Enrollment Coordinator educates on the PACE program, determines if someone is eligible and walks along side of them as they join the innovative program.
What you’ll do
Educates potential members and their advocates Conducts face to face functional assessments at potential member’s homes, or in acute care settings Identifies critical care needs and collaborates with the potential member, their support system, and PACE care team to create a transitional care plan Assists potential members in applying for long term care Medicaid Facilitates conversations to help individuals seamlessly transition into the PACE program Maintains and submits records, including enrollment paperwork, progress notes and Medicaid documentation Links individuals to, and collaborates with, appropriate community agencies and resources Consults with and advises team members as to the relationship of social, emotional, and cultural factors to health and medical care, and as to the availability of services in the community Assists with community engagement and outreach Participates in quality improvement activitiesWhat it takes
Active State of Michigan Licensed Master Social Worker (LMSW)/ Limited Licensed Master Social Worker (LLMSW) Valid license for driving, and reliable vehicle to travel (mileage will be reimbursed) Minimum of one (1) year of experience working with the frail elderly, or long-term care population.Why Brio Living Services
