Shift Monday – Friday 8:00 am – 5:00 pm Job Summary The UK Department of Psychiatry seeks a Social Worker Senior to work with children, adolescents and families and pediatricians in the community to provide support and education related to mental health conditions.The Department of Psychiatry has received funding that is aimed at providing support and education for community pediatricians and PCPs to be educated about pediatric and adolescent mental health, screening for mental health conditions and to have access to consultation for mental health. Working closely with child psychiatrists, this individual will answer calls from pediatricians and then help coordinate a referral for mental health services (to our clinic or to another clinic), assist with tracking the consultations and outcomes, and coordinate with a psychiatrist (or other team members) to answer complex questions about behavioral health conditions and how to handle urgent referrals.
In addition, this individual will be part of a multidisciplinary team of providers to provide comprehensive mental healthcare and support for patients in the Child and Adolescent Psychiatry Ambulatory setting. The Social Worker Senior will be responsible for providing patient education, coordinating patient and family resources and referrals, linking patients to care, providing follow-up, participating in program meetings, and collecting testing and quality data.
This position will also perform and document psychosocial assessments and empower patients toward self-sufficiency. The Social Worker will also coordinate delivery of services to ensure that duplication does not occur and assist with patient care by helping patients and families access available medical and social services either directly or through referrals. The Social Worker will facilitate communication with schools, primary care pediatrics, and other resources. This individual will take a lead role in care coordination for children and adolescents at UK Psychiatry.
