Location: Will be both patient facing and working directly with providers and their staff. Please source from Houston,TX area (30 minute radius)

SHIFT:8am-5pm with some flexibility
No overtime

Duration: 3.+ months, possible extension

Describe the performance expectations/metrics for this individual and their team:
  • Field based position embedded in provider offices to make outreach to patients related to medication adherence, medication reconciliation, education
  • Work with patients on understanding of medications and indication for taking prescribed medications
  • Educate patients on medication purpose, interactions, importance of medication adherence
  • Identify patient barriers to obtaining medications, Refer patients as indicated to additional care team members for access to resources
  • Occasional meet with patients in place of residence, inpatient or post acute setting for medication education and medication review
  • Communicate effectively with providers related to patient medication related issues/barriers/concerns
  • Close program care gaps related to medication adherence
  • Report out to physicians and physician leadership on findings and recommendations for quality improvement
  • Work with Care Team to support patient needs, access to care, clinical program and community resources
Required Skills/Experience: Preferred Skills/ Experience: 1. Experience working with a health plan/IPA/Provider facing 1. Medicare experience preferred 2. Does need to have good people and communication skills 2. Bilingual Highly Preferred 3. Prefer 2+ years in pharmacy or facility with direct patient contact Education Requirement: High School/GED Education Preferred: 2+ yrs Software Skills Required: Word, Excel Required Certifications: Pharmacy Tech Required Testing:
