What you'll do:

Conduct outreach to patients from diverse populations to engage them in population health initiativesBuild rapport and establish trusting relationships with patients to promote their active participation in careIdentify barriers to healthcare access and address social determinants of health that impact patient well-beingEducate patients on available resources, preventive measures, and self-care strategiesPerform comprehensive medical and behavioral assessments to evaluate patients' physical, psychological, and social needsIdentify and address mental health issues, substance abuse, domestic violence, and other psychosocial factorsCollaborate with healthcare providers to develop individualized care plans based on assessment findingsAdvocate for patients' needs and preferences in care planning and decision-making processesCollaborate closely with a multi-disciplinary team, including physicians, nurses, care coordinators, and community partnersParticipate in care conferences and team meetings to discuss patient cases and develop integrated care plansShare expertise on social determinants of health and provide guidance to the team on addressing patients' social and emotional needsCoordinate referrals to community resources, mental health services, and support groupsMaintain accurate and up-to-date documentation of patient interactions, assessments, and care plansAdhere to privacy regulations and ensure the confidentiality of patient informationProvide timely and thorough reports to the multi-disciplinary team regarding patient progress and interventionsCollaborate with healthcare providers to ensure continuity of care and coordination of service Requirements Master's degree in Social Work (MSW) from an accredited institutionCurrent and valid licensure as a Licensed Social Worker (LSW) or Licensed Clinical Social Worker (LCSW)Minimum of 2 years of experience as a social worker, preferably in a healthcare or population health settingKnowledge of medical and behavioral assessment techniques, mental health interventions, and crisis managementStrong understanding of social determinants of health and their impact on patient well-beingExcellent communication and interpersonal skills to engage patients and collaborate with a diverse teamDemonstrated ability to work collaboratively in a multi-disciplinary team environmentStrong interpersonal skills to build effective relationships with colleagues, patients, and community partnersExcellent problem-solving and conflict resolution skills
