A. Position Definition & Summary:

The Transition Services Manager will provide oversight of respective case managers and the vocational program manager, provide reintegration services from the beginning to the end of the program including assessment, self[1]sufficiency planning, coordinating with community networks, and providing a seamless connection and transition for the youth. This includes providing compliance monitoring for 60 days following release from the program to ensure the youth is meeting all criteria of his transition plan in collaboration with the youth, family, JPO, and community support services

B. Position Expectations and Essential Functions:

Participates in and provides input into formal treatment team regarding youth’s transitional activities and self[1]sufficiency planning.

Completes a self-sufficiency plan with youth and family and incorporates developed goals into the youths Individualized Performance Plan.

Meet with each applicable youth on an individual basis to assess independent living skill needs and identify existing transition needs, new transition needs and progress towards goals.

Provide service delivery, in conjunction with the Program Vocational Manager, of Overcoming Obstacles to all youth in the facility to address individualized skills for prevocational, vocational and life skills.

Will be crossed trained on Case Manager skills to include RAY and IOC.

Work collaboratively with case manager to develop an individualized Transition Plan with each youth to include the input of all entities involved with this case, to target the right services at the right location, at the right time for each youth.

Facilitate mock interviews with youth and ensure they are provided the opportunity to practice with interviewing attire. Works collaboratively with the onsite Recreation Therapist to ensure inclusion of identified leisure interests into the youth’s exit portfolio.

Participate on the youth’s Circuit Transition Panel (if assigned) in his home circuit for discussion of his recommended needs upon release and the resources available in that area.

Maintain a system to ensure those needs and services identified by the Circuit Transitional Panel are followed.

Pursue home community support resources for the family, in the event a crisis occurs with the youth’s family (food bank, churches, etc.)

Assist and secure living arrangements for the youth:

a. If notified of a youth who is younger than 18 years old and cannot live independently, the TSM will contact the Department of Children (DCF) and Families to identify foster home placement. Once a foster home is identified, the TSM will include the foster care parent in treatment team meetings.

b. For a youth who becomes homeless while in the program, the TSM will initiate communication with DCF in his home region and will work together with the DCF staff to identify an alternative living situation. The TSM will place preference on locating a relative who can provide housing conducive to the needs of the youth.

c. For a youth who is under 18 years old while in the program, but will be at least 18 years old by discharge, the TSM will initiate the petitioning process with the Court to extend jurisdiction up to a year and will assist the youth throughout the process.

d. For a youth who is at least 18 years old and homeless, the TCM will provide him a list of community resources for temporary housing. Examples of community organizations are the Salvation Army and Metropolitan Ministries

Provide each youth with a comprehensive Transition Portfolio completed by the TSM as applicable to the needs of the youth. Examples of documents that may be included are:

a. Education: Education transcript, GED or Diploma, certificate of completion; financial aid applications, including Florida Juvenile Justice Foundation; letters of recommendation from program and education.

b. Social Services: The TSM will make diligent efforts to obtain a copy of the youth’s birth certificate, social security card, State card and any resources available under , that may be additional support to the youth.

c. Employment: The TSM will maintain industry-recognized certifications, Florida Ready-to-Work credentials, resume sample and/or any additional pre-employment documentation that will assist the youth in future jobs. These will be part of the youth’s Transition Portfolio that will be in his possession upon discharge from the facility. In addition, the TSM will provide the location of Career Source centers in the youth’s home community.

d. Healthcare: The TSM will ensure appointments are in place for Determination of Medicaid eligibility and application. In addition to applicable healthcare screenings. The youth’s portfolio will include contact information for care review teams (under interagency agreement) if applicable.

e. The TSM will serve as the liaison between the program and Project Connect or Project Bridge, if youth is referred, to ensure proper planning at discharge.

Monitor risk changes throughout the course of stay as they relate to transition needs.

Provides compliance monitoring for 60 days following release from the program to ensure the youth is meeting all criteria of his transition plan in collaboration with the youth, family, JPO, and community support services.

The TSM will assist the Community Relations Director with obtaining and maintaining mentors.

Chronological documentation to be maintained for all contact as well as completion of a summary when ending post release follow up work.

Effectively implements and consistently applies the program’s Positive Performance System (PPS) / Behavior Management System (BMS).

Detects, responds, and prevents abuse of youth.

Performs other duties as assigned by supervisor.

Qualifications

C. Position Requirements:

Education/Experience:

A bachelor’s degree from an accredited university or college in the field of social work, psychology, sociology, criminal justice, or related human services field and at least two (2) years of experience in case management or service coordination.

Other Requirements:

Appointees will be required to possess a driver's license valid in the state where the work site is located only if the operation of a vehicle, rather than employee mobility, is necessary to perform the essential duties of the position.

D. Knowledge, Skills and Abilities:

College level reading ability, oral communication skills, and written communication skills to include legible handwriting.

Basic knowledge of human behavior, principles of psychology, human growth and development, and the emotional needs of residential youth. Experience with therapeutic limit-setting or an understanding of level systems or other behavior management modalities, helpful.

Ability to teach or assist with youth’s activities of daily living.

Ability to demonstrate judgment and discretion in dealing with sensitive information and maintain all information regarding the facility, youth, and other staff as highly confidential.

Ability to deal courteously and maintain professional boundaries with youth and other staff and the willingness to seek counsel from supervisory staff in this regard.

Ability to tolerate verbal, mental, and occasional physical abuse while maintaining a professional demeanor.

Ability to use appropriate social skills with youth and other staff, including verbal, nonverbal, written, and visual demonstrations of effective communication, conflict resolution, active listening, empathy, healthy relationships, and respect.

Ability to function independently within a team-oriented atmosphere, work under pressure, meet inflexible deadlines, set priorities, and deal diplomatically with difficult individuals.

Ability to deal courteously with the public and use appropriate customer service skills through electronic, telephonic and face to face interactions.

Ability to prioritize and manage time effectively, display strong multi-tasking capabilities and use appropriate problem-solving skills to develop solutions under minimal supervision when necessary.

Ability to maintain all TrueCore and DJJ annual training and certification requirements.

F. Physical Requirements & Work Environment:

Processing and Reasoning : Ability to exercise self-control in potentially volatile situations such as in verbal and physical confrontations. Must be able to work and concentrate amidst distractions and noise. Ability to assist youth with problem-solving and maintaining self-control in volatile situations.

Language : Ability to write complex sentences, using proper punctuation, and ability to communicate in complex sentences using normal word order with present and past tenses.

Talking : Ability to express and exchange ideas via spoken word during activities in which they must convey details or important spoken instructions to others accurately, sometimes quickly and loudly.

Hearing : Ability to perceive the nature of sounds with no less than 40 dB loss at Hz, 1000 Hz and 2000 Hz with or without correction. Ability to receive detailed information through oral communication. Usual noise levels range between barely audible (20dBa) to heavy noise (80dBa) as the primary work environment is a residential type setting where an individual lives and services are available from a provider on a 24-hour basis.

Vision : To complete required clerical tasks and the observation of youth, a minimum visual acuity standard of 20/70 in either eye or both eyes together with or without corrective lenses is required. However, if one eye is blind or 20/200 or worse, the other eye must be 20/40 or better.

Dexterity: Ability to stoop, kneel, crouch, reach, push, pull, lift, grasp, and be able to perceive the attributes of an object such as size, shape, and temperature. Movements frequently and regularly required using wrists, hands and/or fingers.

Physical Strength : Ability to exert up to 50 pounds of force occasionally. Position is mainly sedentary work (office work) but may on occasion include physical force execution. The employee must be able to physically perform escalation matrix as defined in Right Interactions. This will include but is not limited to all countermoves and restraint techniques.

Climate : The employee is not substantially exposed to adverse environmental conditions as the work is predominantly inside however the employee may be subject to temperature changes as the employee may be required to work both inside and outside in heat/cold, wet/humid, and dry/arid conditions.

Universal Precautions : The employee will be required to observe Universal Standard precautions to include the use of personal protective equipment (PPE) to prevent contact with potentially infectious materials such as blood, vomit, saliva, sputum, urine, and feces.
