Position Summary: The Substance Use Navigator (SUN) is a care coordinator and an integral team member of a statewide initiative to improve access to care coordination and medication for Opioid Use Disorder. The SUN conducts initial assessments, introduces patients to substance use programs and services, expedites and provides access to outpatient clinics and treatment, serves as a coach for patients, and maintains ongoing contact with their patient panel. Additionally, the SUN assists with access to other services such as financial counseling, primary care, mental health services, social services, and residential treatment facilities. The Sun is responsible for interacting and working with the health care team and with patients in the hospital and Emergency Department to find and connect patients to drug treatment resources and outpatient clinic referrals. The SUN acts as a liaison to state or other local programs aimed at providing assistance and resources for patients with substance use and abuse disorders. 

Communicate with hospital staff to recruit patients and provide support to staff around medication assisted treatment (MAT): Monitors the ED and inpatient patient tracking systems to screen for eligible patients; checks in with ED and inpatient clinicians and nursing staff to screen for eligible patients. Identify current physician/clinic and behavioral health providers. Works to connect patient with existing MAT-capable clinics for treatment available at current primary care and/or behavioral health location if available, unless patient prefers an alternate medication assisted treatment location. Identify primary medical home if not secured. Maintain updated, in-depth knowledge of local and regional substance use disorder treatment programs including capacity, insurance requirements, and additional services provided at local and regional programs (benefits assistance, housing assistance, mental health, primary care, etc.). Assess and address any barriers to attending follow-up appointments (transportation, contact phone # availability for appointment reminders, shelter needs, etc.). Engage social services to assist with any of these issues as needed. Arranges transportation to nearby residential treatment facilities and partner programs as needed Assist with navigating barriers to patients obtaining buprenorphine prescription from pharmacy (insurance status, copay expense, cost differences between formulations, etc). Routinely assists patients by having the patient and/or pharmacist call from the pharmacy to sort out encountered insurance barriers in real time. Qualifications

Minimum two years’ experience in a drug treatment environment required

Recent inpatient drug treatment experience preferred

Recent inpatient hospital experience preferred

Associate’s degree in a health care related field required

Bachelor’s degree in a related field of substance abuse, crisis management, or counseling preferred

COMMUNICATION SKILLS:

Ability to read, write, and speak effectively in English, required

Bi-lingual preferred

Respect for patient confidentiality and privacy required
