Description:

As a Registered Nurse in the Community Health/Home Health unit, you will provide high-quality nursing care to patients in their homes. This role involves assessing patient needs, developing care plans, and collaborating with other healthcare professionals to ensure effective home-based care.

Key Responsibilities:

  • Patient Care: Provide, teach, supervise, and document nursing care for patients in their homes.
  • Health Promotion: Intervene to promote, maintain, and restore health, prevent illness, and effect rehabilitation.
  • Care Coordination: Collaborate with healthcare providers, professionals, and community representatives to coordinate home care services.
  • Assessment: Evaluate patient and family responses to interventions and adjust care plans as necessary.
  • Independence: Function independently under general guidance from the Director, following unit-specific guidelines, protocols, and policies.
  • Critical Thinking: Utilize time management, communication, planning, delegating, and problem-solving skills in providing care.
  • Documentation: Maintain accurate records and communicate clinical data both verbally and in writing.
  • On-Call Responsibilities: Be willing to accept “on-call” responsibilities as needed.
  • Flexibility: Demonstrate flexibility in work hours and assignments as required by the role.

Requirements:

  • Experience:
    • One year of acute nursing care experience is required; hospital-based experience is preferred.
    • Experience in home health is required.
  • Certifications & Licenses:
    • Certified Home Health Nurse including proficiency with OASIS is required.
    • Infusion Therapy Certification is preferred.
    • BSN is strongly preferred.
    • Current Driver’s License and Insurance are required.
    • COVID-19 Vaccination is required (dose date on or after 10/3/23) or an approved exemption form. If exempt, a mask must be worn at all times on-site.
    • Massachusetts Temporary Nursing Service Agency License (Nursing Pool) must be uploaded with profile for consideration.
  • Skills:
    • Demonstrated knowledge of community health principles and practices.
    • Ability to perform independently as a clinician with minimal supervision.
    • Proficiency with self-scheduling of caseload is preferred.
    • Knowledge of home care reimbursement and regulatory guidelines.
