JOB DESCRIPTION SUMMARY :
Under limited supervision, The Access Center RN is responsible for the management of patients who are pending admission to a gilchrist program. This position is responsible for following up on the patients whose admission is delayed and facilitating a timely resolution, so patients receive needed care sooner. This position maintains a data base of these patients which is used to trend and report the reasons for delays in care, suggest changes to workflow and/or program development opportunities to leadership regularly to allow for earlier care intervention for our patients and referrers. This position participates as a member of the Leadership Team and assists with the ongoing development of the Field Access RN’s and coordinators. Education
Diploma, ADN and/or BSN from an accredited institution
Experience
Two to four years of nursing. Hospice admission experience preferred.
Knowledge, Skills and Abilities
Strong time management and organizational skills. Excellent interpersonal and communication skills with ability to interact with a diverse patient population, and advanced practice nurses and physicians. Ability to perform well in a busy work environment consistently overcoming objections with professionalism with both referral sources and patients. Strong and creative, problem-solving skills with the ability to prioritize and delegate. Analytical skills necessary to organize and oversee large amounts of data and to influence the work of others. Advanced skills and knowledge of admission and eligibility requirements for all gilchrist programs and services, as well as strong customer service skills. Effective planning and organization skills to manage multiple priorities simultaneously. Strong analytical skills to identify and develop opportunities for workflow changes, educational opportunities for staff, and trending of data to suggest new program opportunities. Effective skills in use of computer programs. Demonstrated skill in managing complex service issues, service recovery. Able to delegate assignments and provide direction to access center and access nursing personnel. Comfortable with working with a high level of autonomy.Licensures, Certifications
Current Maryland State license as a Registered NursePhysical Requirements
Ability to concentrate and pay close attention to detail often in stressful situations. Ability to perform home care visits Generally sedentary work but may require standing and walking for approximately 10% of work timeWorking Conditions
