POSITION:

Nurse Coordinator

POSITION PURPOSE:

Provide direction, coordination, supervision and implementation of professional and supportive services to Home Care Agency (HCA) consumers. Monitor services provided to HCA consumers, provide training to attendants and ensure consumer satisfaction and safety. Complete and submit all required documentation.

Key Duties and Responsibilities include:

Communication with team and clients
Medical documentation Annual In-Person Supervisory Visits 
Perform record audits Education to clients, employees and families
Utilizing Carevoyant software program Requirements

Experience:

1 year experience practicing as an RN (required) with additional experience in acute care  Med/surg experience highly preferred Working knowledge of disabled community is preferred. A person with a "disability" is someone who has a physical, mental or communication disability that substantially limits one or more of the major life activities such as caring for oneself, performing manual tasks, walking, seeing, hearing, speaking, breathing, learning and working. 1 year of supervision experience overseeing caregivers and other care providers Spanish speaking (highly preferred)

Education and/or Licenses:

Current, unencumbered state certification to practice as an RN in the state of Colorado BSN highly preferred Current CPR / First Aid certification

Pay Range: $33 - $36.50/hour DOE + $2,000 sign-on bonus

Location:  This position will have a territory of Lakewood, Littleton, Denver metro
Work Environment: Hybrid (Tu/Th in office; M/W/F remote)
