Job Summary:

The Recovery Navigator in the Bay Cove Behavioral Health and Wellness Center will play a critical role in the successful implementation of the Bay Cove Behavioral Health and Wellness Center by working with members to overcome barriers and stigma and by assisting members to understand their medical and behavioral health system of care. This position contributes to Bay Cove’s mission by working in a program providing access to services that address the needs of community members most impacted by deleterious social determinants of health.

This is a non-exempt position.

Essential Functions of the Position: 

Provides brief interventions in community settings to educate Bay Cove Behavioral Health and Wellness Center client’s on the availability of behavioral health services and assist members with navigating into a sustainable system of care 

Educates and empowers members on behavioral health interventions

Serves as a liaison between clients, healthcare providers and human service agencies to reduce barriers to care and assures clients receive the care they need when they need it

Conducts assessments of health risk and service needs including an assessment of members’ strengths, preferences and preferred family and community supports

Develops a member directed service plan and documents the plan in the EMR 

Performs assessment of member’s eligibility for entitlement programs (ie MassHealth, SSI,SSDI) and assists member with making linkages to a sustainable system of care

Organize and facilitate the effective functioning of the Bay Cove Behavioral Health and Wellness Center care team, including coordinating meetings, facilitating communication and documentation

Drive referrals regarding connections to any community or social services that align with the member’s needs and goals

Provides coaching and support to clients as needed

Other responsibilities as assigned by the supervisor

Job Requirements: 

Bachelor's degree in social work, psychology, counseling or related field required.

A High School diploma or GED plus two years direct care experience providing services to adults and families with mental health issues or related populations can be substituted for the above degree requirements with a preference for individuals with lived experience.

Experience in working with the target population of the Bay Cove Behavioral Health and Wellness Center; uninsured and underinsured individuals with serious mental health challenges and/or substance use disorders

Professional writing and oral communications skills.

Possesses knowledge of substance abuse assessment, recovery principles

High degree of personal organization, attention to detail, and time management skills.

Proficiency in word processing and other computer functions using Windows, Word, and Google Email and Apps.

Use of personal cellular telephone for work communication.

Bilingual candidates encouraged to apply, pay differential offered for candidates with fluency in certain essential languages.
