This position is eligible for the Education Debt Reduction Program (EDRP), a student loan payment reimbursement program. Graduate of a school of professional nursing approved by the appropriate accrediting agency and accredited by one of the following accrediting bodies at the time the program was completed by the applicant: The Accreditation Commission for Education in Nursing (ACEN) or The Commission on Collegiate Nursing Education (CCNE); OR Individuals attending a master's level bridge program in nursing who have completed coursework equivalent to a bachelor's level degree in Nursing may have opportunity to become registered as a nurse with a state licensing board prior to completion of the bridge program. Upon achievement of a State license, the individual may be appointed on temporary basis and later converted to a permanent appointment upon successful completion and graduation from the bridge program. Current, full, active, and unrestricted registration as a graduate professional nurse in a State, Territory or Commonwealth (i.e., Puerto Rico) of the United States, or the District of Columbia. Graduate Nurse Technician (GNT) Exception: Candidates who otherwise meet the basic education requirements, but do not possess the required licensure, may be appointed at the entry step of the grade and level applicable to the completed nursing education as a GNT on a -day temporary appointment while actively pursuing licensure (may be extended up to two years on a case-by-case-basis.) NOTE: Grandfathering Provision - All persons currently employed in VHA in series and performing the duties as described in the qualification standard on the effective date of the standard (1/29/) are considered to have met all qualification requirements for the grade held including positive education and licensure/certification. There is a large outpatient psychiatric population, with admissions for medical co-morbidities. This position requires visual acuity (glasses permitted), keen hearing (hearing aid permitted), clear distinctive speech, and manual dexterity. This position requires potentially long periods of continued walking (up to 8 hours), standing (up to 8 hours), stooping, sitting, bending (up to 8 hours), pulling, and pushing. Care Coordinators emphasize collaboration and engage in processes that assess, advocate, plan, implement, coordinate, monitor, and evaluate health care options and services so that they meet the needs of the individual patient. They are accountable for utilizing critical thinking skills and sound judgement to ensure positive outcomes and deliver quality patient care virtually. Care Coordinators develop and coordinate seamless transitioning of patients into the Home Telehealth program and discharge back into the PACT/CBOC. They will provide support at the PACT/CBOC, service line, and organizational levels.Care Coordinators provide initial and ongoing comprehensive assessment to include a review of systems which establishes a comprehensive plan of care. Triage and assess all data received from Home Telehealth patients such as vital signs, reported symptoms and question responses. Identifies and intervenes for potential exacerbations of complications to facilitate timely care in clinic, ER/urgent care, and or/care in the community. Care Coordinators through the treatment plan assesses, identifies, analyzes and prioritizes problems, interventions and appropriate measurable goals (i.e., Simple, Measurable, Achievable, Realistic, Time-oriented (SMART) goals). The treatment plan is an extension of the Veteran's primary care plan and is completed in collaboration with PACT, HBPC, Mental Health and other specialty care services as appropriate for each Veteran enrolled in the program.
