Description

Position Summary: Plans, organizes and coordinates the operations of the program by performing the following functions directly or through delegation to appropriate subordinate staff members. Additionally responsible for providing case management services to clients.

Principle Responsibilities :

1. Administration and delivery of client services and supervise staff.

2. Provide information and referrals to potential clients.

3. Provide case management services, including intake, assessment, care planning, application assistance and advocacy, to clients of SHARP.

4. Conduct individual and group caregiving services to clients and families.

5. Provide support groups for caregivers.

6. Provide training to staff and volunteers of SHARP.

7. Assist the Senior Programs Director in ensuring that the program is in compliance with all private and public funders, including SOFA, NYC Aging, and NYS Dept. of Health, and Selfhelp.

8. Interview and evaluate prospective clients and their families or caregivers as to their appropriateness for the program, and maintains ongoing contact.

9. Handle problems and emergencies with solid professional judgment. Confers with the Senior Programs Director, as appropriate.

10. Maintain case notes, records, and required documentation, according to departmental standards and/or regulatory requirements.

11. Prepare mandated statistics and other necessary paperwork. 

12. Consult with and ensure the Senior Programs Director is informed of activities, needs and problems related to clients, staff and program.

13. Complete appraisals for Sharp program assistants, and PSRLs.

14. Conduct Memory Screening Program four times annually.

15. Assists with outreach and training components of the program, including presentations to community organizations and trainings to providers.

16. Performs other related duties as assigned or requested.

17. Adheres to organization’s standards by completing annual mandatory trainings in a timely manner on topics including – but not limited to – Sexual Harassment Prevention Training, IT Security Awareness, Compliance, OSHA, HIPAA and any other training the organization may deem necessary in regard to regulatory compliance or good business practices.

18. Supports organization’s mission by striving for excellence in all aspects of their job with a focus on positive interpersonal relationship with co-workers.

19. Adheres to the organization’s policy in regard to absenteeism and appearance and health and safety standards.

Job Competencies & Minimum Qualifications :

Master's degree in Social Work (MSW) or related field, and two years of human services experience, preferably in the field of aging; or Bachelor’s degree in related field and four years of human services experience NYS license and registration to practice as a Licensed or Certified Social Worker in New York State preferred May supervise the social work and/or support staff in the program; interviewing and training employees; planning, assigning, and directing work; appraising performance; rewarding and disciplining employees; addressing complaints and resolving problems Computer literate Excellent verbal and written skills Excellent customer service skills Able to multi-task and work independently with a great attention to detail

Working Conditions/Physical Demand :

Business office environment with phone and computer use. Must frequently lift and/or move up to 10 pounds and occasionally lift and/or move up to 25 pounds. Occasional exposure to outdoor weather conditions, and risk of exposure to pathogens and unsanitary conditions in client’s homes. Must be able to attend meetings, other out-of-office functions and travel to client homes by using public or private transportation or other conveyance. Valid driver’s license, car registration and automobile insurance, as applicable.
