Forensic Case Manager - MHP001 - Mon-Fri 9AM-5PM

Position Summary : The Forensic Case Manager is responsible for the daily operations of the Supported Housing Prison Reentry. Provide case management services, monitor consumers’ compliance with release plans, coordinate services with family members and collaterals, provide monitoring reports to administrative and legal authorities, ensure quality of care to the consumer and assist each individual in maintaining his/her independent living environment. This position may have supervised / unsupervised contact and or conversational interaction with consumers while working in the residences, day treatments and habilitation facilities.

Essential Accountabilities :

Responsible for the operation of the Supported Housing Prison Reentry program and assigned caseload. Daily travel throughout New York City to providers, consumer residences, and other human service agencies as well as occasional travel to statewide meetings. Travel is done throughout the year, including during adverse weather conditions to assess, observe and evaluate physical plant and consumers’ functioning abilities and needs. Act as a liaison with the Reentry Coordination System (RCS), parole officer, community health, mental health, educational, vocational, and recreational programs. Participate in discharge/transition and interagency planning. Facilitate ongoing crisis management, wellness and harm reduction groups. Obtain random urine toxicology on-site or through clinical providers. Provide linkage services to detoxification and rehabilitation treatment centers as needed. Assist consumers with linkage to community support services in identified areas of need. Assist consumers in the housing selection process including participation in the selection of adequate furnishings, working with real estate agents securing service from utility companies (telephone, electric), etc. Facilitate sublease and rental payment agreement. Familiarize the consumer with his/her rights as a tenant and assist in tenant/landlord communication. Streamline benefit applications; facilitate housing subsidies, food stamps, other benefits and entitlements. In conjunction with consumer, develop individual support plan and to address areas such as; self-care (, maintenance of sobriety), money management, medication management, daily living skills and symptom management. Provide Evidence Based Practices (EBP) that is the most current best practices, utilizing clinical expertise, as well as the preferences of the consumer. Maintain all required written documentation including consumer records, program records. Identify maintenance and safety needs of assigned apartments. Responsible for reporting and documentation of all incidents to immediate supervisor and/or the Director of MHS within 24 hours. On-call 24 hours a day, seven (7) days per week to respond to emergencies. Weekly supervision of assigned staff/ peer specialist, to delegate tasks and provide professional direction and methodological assistance. Able to lift/carry packages and boxes of supplies; as well as clients if necessary. Communicate information with all staff and supervisor(s) verbally and in writing for the purpose of giving instructions, providing feedback and adjusting work practices. If deemed necessary must be able to participate in these programs at various sites and varying times. The ability to complete the following training offered and perform the techniques taught in each area a required function of the position; AMAP, CPR and First Aid. Participate in activities of other staff members in their absence or during periods of staff shortage.  Represent the agency at meetings, trainings not otherwise specified.  Ability to work flexible schedule as work schedules are subject to change.  This does not limit the assignment of duties or exclude the performance of other duties not mentioned, as assigned by supervisor. Forensic Case Manager - MHP001 - Mon-Fri 9AM-5PM

Bachelor’s degree in Social Services (ie: social work, psychology) required (MSW preferred), in addition to a minimum of (3) experience working with mental health consumers with/without co-occurring addictive disorders and forensic history. A minimum of (2) years supervisory experience. New York State driver’s license and access to a vehicle required.
