WellLife Network Inc. is seeking a compassionate Senior Case Manager for our Apartment Treatment Program. The ideal candidate will provide case management services to individuals with Mental disabilities. Full-Time opportunity with excellent benefits. Key Responsibilities:
  •  Supervise 3 – 4 case managers. 
  •  Monitors cases by verifying clients' attendance.
  •  Observing and evaluating treatments and responses
  •  Advocating for needed services and entitlements.
  • Obtaining additional resources; intervening in crises
  • Providing personal support. 
  •  Maintains clients' records by reviewing case notes. 
  • Logging events and progress.
Qualifications:
  •  High School Diploma or Equivalent with Five (5) years of experience preferably supervising Case Managers within a mental health setting.
OR
  • Associate’s degree in human services or related field with at least Two-Three (2-3) years of experience supervising Case Managers within a mental health setting.
OR
  • Bachelor’s degree in human services with at least One-Two (1-2) years of experience supervising Case Managers within a mental health setting. 
  •  New York State driver’s license and access to a vehicle required.
