Medication Reconciliation Pharmacy Technician

Position Summary:
The Medication Reconciliation Pharmacy Technician, under the supervision of a pharmacist, and according to departmental policies, procedures and guidelines, will assist pharmacists in obtaining an accurate patient's medication history. The Medication Reconciliation Pharmacy Technician is responsible for accurately interviewing, collecting and entering current patient medications into the hospital information system record for the purpose of facilitating medication reconciliation by the admitting provider.
Minimum Requirements:
Technician Level I
Education: High school graduate or equivalent. Refer to the requirements of the Massachusetts Board of Registration in Pharmacy Regulations.
Licenses/Registrations: Current registration as a pharmacy technician or technician trainee with the Massachusetts Board of Registration in Pharmacy.
Experience: Health-system pharmacy experience preferred, but not required.
Technician Level II
Education: All requirements for level I and successful completion of the HMC Technician Training program.
Licenses/Registrations: Current registration as a pharmacy technician with the Massachusetts Board of Registration in Pharmacy. An active national certification as a pharmacy technician (e.g.: PTCB).
Experience: At least 500 hours of experience in health-system pharmacy.
Technician Level III
Education: All requirements for levels I and II, plus successful completion of the HMC Intravenous Technician Training program.
Licenses/Registrations: Current registration as a pharmacy technician with the Massachusetts Board of Registration in Pharmacy. National certification as a pharmacy technician (e.g.: PTCB).
Quality Assurance: Participation with and maintenance of negative media fill and fingertip testing at required intervals.
Experience: At least 500 hours of experience as a Technician Level II.
Technician Level IV
Education: All requirements for prior levels, plus successful completion of the HMC Chemotherapy Technician Training program.
Licenses/Registrations: Current registration as a pharmacy technician with the Massachusetts Board of Registration in Pharmacy. National certification as a pharmacy technician (e.g.: PTCB).
Quality Assurance: Participation with and maintenance of negative media fill and fingertip testing at required intervals. Participation in and maintenance of health screenings.
Experience: At least 1000 hours of experience as a Technician Level III.
