Schedule: Monday-Friday, 8a-4:30p 

FTE: 1.0 (40/hrs per week), Non-Exempt

Minimum Pay Rate: $22/hr 

Provides technical nursing care to acute and chronically ill children from the age of birth to 21 years and selected adults patients, under the direction of a RN Care Manager, and in accordance with the Ohio Board of Nursing Law and Rule. (Sections 4723-4-01, Ohio Administrative Code or OAC, 4723-4-04, OAC, 4723-4-06, OAC, and 4723-4-08 OAC). Demonstrates competence defined (*) in LPN II responsibilities in providing age appropriate care and service to pediatric patients from birth to 21 years, and selected adults.

Responsibilities

1. Identifies and documents patient/family objective/subjective data and family data and observations. Reports pertinent findings to the RN Care Manager responsible for directing the patient's care.

2. Follows the plan of care appropriate to the developmental level of the patient.

3. Collaborates with the RN Care Manager directing the patient's care regarding appropriate tasks/nursing interventions.

4. Implements nursing interventions according to established standards demonstrating competence in specified nursing care.

5. Administers and documents medications by all routes, i.e. oral, subcutaneous, intramuscular, intradermal, transdermal, rectal and aerosol, utilizing the five rights and all other related safety recommendations.

6. Demonstrates basic written and verbal communication skills to convey nursing actions and patient?s/family response.

7. Demonstrates sound clinical decision making.

8. Communicates and documents patient response to care.

9. Provides input into revision of plan and care based upon data collected and identified patient/family needs.

10. Following additional education, training, and demonstrated competency, selected LPN II's may perform venipunctures for the purpose of drawing blood only. (not done in the inpatient units)

11. Following education, training and demonstrated competency, LPN II's may perform the following IV therapy procedures:

a. Verification of the type of peripheral IV solution.

b. Examination of a peripheral infusion site.

c. Monitor and document the hourly amount of peripheral IV infusion according to the prescribed flow rate. (Rate changes are limited to RNs only.)

d. Reset pumps and silence alarms.

e. Discontinuation of a peripheral IV device.

f. Performance of routine dressing changes at the insertion site of a peripheral infusion, a peripherally inserted venous catheter that has been sutured, or central line catheter.

g. Assembles tubing for both peripheral IVs and central venous catheters ( does not include spiking bag or priming tubing).

h. The following practices are prohibited from LPN practice:

Initiating and Maintaining: 1) Blood and blood components; 2) Solutions for total parenteral nutrition; 3) Any cancer therapeutic medications; 4) Any investigational or experimental medications; or 5) Arterial lines

i. LPNS may not initiate but may maintain: Solutions that are administered through any central venous line, umbilical catheter, and peripherally inserted venous catheter.

12. Maintains accountability for own actions in completing tasks within the LPN scope of practice.

13. Acts as a patient/family advocate.

14. Demonstrates positive, effective, and age appropriate interaction skills with patient/family, physicians and other health care team members.

15. Provides instructions and explains procedures to patients/families according to established standards.

16. Reinforces teaching related to diagnosis and home-going instructions.

17. Assists with examinations and treatments.

18. Uses established channels of communication to seek assistance, obtain clarification, share information and offer suggestions.

19. Demonstrates respect and understanding of the roles of other health care team members in the provision of patient/family care.

20. Assists in emergency situations as directed by the RN Care Managers.

21. Develops a beginning awareness of research, participates in unit based research and evidence based practice evaluations.

22. Collects pertinent data.

23. Participates in unit quality improvement activities.

24. Participates in assessing own educational needs.

25. Formulates annual goals and develops a plan to accomplish them.

26. Pursues learning opportunities through attendance at inservice programs, continuing education, reading and/or formal education programs.

27. Performs other duties as assigned.

Qualifications

KNOWLEDGE , SKILLS AND ABILITIES REQUIRED:

Must be a graduate of an approved school of practical or vocational nursing.

Must be licensed as a Practical Nurse in the State of Ohio.

Must complete an Ohio Board of Nursing approved medication test, such as NAPNES (National Association for Practical Nurse Education and Service) or OAPNES (Ohio Association for Practical Nurse Education Service.)

Must satisfactorily complete orientation objectives as determined by the manager.

MINIMUM PHYSICAL REQUIREMENTS: Must be able to see, hear, stand, walk, speak, read and perform manual tasks with or without accommodation, and care for oneself with little or no difficulty. Must be able to independently lift up to 25 lbs. Must be able to move or reposition patients of any weight or size with the assistance of another person(s) and/or equipment.
