Sarah’s House is a supportive housing program offering emergency shelter and project-based supportive housing with an array of other services for families experiencing homelessness in Anne Arundel County.
Catholic Charities of Baltimore Sarah's House is current seeking a Case Manager who provides intensive case management including, but not limited to, as-needed home visitation services to participants / clients. The Case Manager assesses household needs, coordinates resources based on goals, and provides advocacy, information, and referral services. Schedule: Monday - Friday: 9am - 5pm
*Must be able to work one evening shift per week (12 pm - 8 pm)*
JOB DUTIES & RESPONSIBILITIES:
Work with clients to primarily focus on improving quality of life in obtaining stable and permanent housing, goal setting, case planning and personal development Maintain casework files on each client according to policies and procedures and organize and record information into ETO and HMIS in a timely manner Provide support, advocacy, information and referral services to Clients of Sarah's House Participate in and accurate track case management progress for 30/60/90 team meetings in HMIS Motivate and empower clients to establish and reach goals, including obtaining information from clients such as: client’s interests, aptitudes, and abilities for developing case plans Ensures client confidentiality is maintained in accordance with HMIS guidelines and requirements. Other duties as assigned.
EDUCATION AND EXPERIENCE REQUIREMENTS:
PHYSICAL REQUIREMENTS & WORK ENVIRONMENT:
Must be physically capable of moving throughout buildings that have stairs Driving (Agency and/or personal vehicle) is required for this position. Driver must be at least 21 years of age, with a minimum of 2 years of driving experience, and a valid driver’s license issued by the state of residence.
