Job Title: Case Manager

Job Summary:

We are seeking a skilled Case Manager to join our team at WestCare Foundation. The successful candidate will be responsible for providing case management services to clients enrolled in transitional living services.

Key Responsibilities:

  • Deliver psychoeducational group services, case management services, education, and community outreach services as necessary.
  • Assess and periodically assess individual needs for case management services.
  • Assist clients in transitions between levels of care.
  • Develop and periodically revise case management plans that include service activities.
  • Communicate, coordinate, refer, link, and perform related activities.
  • Monitor service delivery to ensure client access to necessary services.
  • Provide patient advocacy, linkages to physical and mental health care, transportation, and retention in primary care.
  • Maintain client files according to company policy and contract requirements.
  • Develop and maintain working knowledge of available community resources and act as liaison.
  • Maintain relationships with referring and supporting agencies and represent the agency at various community meetings as requested.

Requirements:

  • Two years of case management experience preferred.
  • Basic computer skills.
  • Good documentation skills.
  • Ability to multi-task.
  • Excellent communication skills (written and oral).
  • Graduation from an accredited four-year college with a degree in Psychology, Social Work, Rehabilitation Counseling, or a related field is preferred.
  • High School Diploma required.

Working Conditions:

Work is primarily performed in an office or in a residential/outpatient treatment setting. Some outdoor activities may be required. UA testing of clients is required. Must be able to de-escalate situations in the program.
