The RN Case Manager establishes and maintains an efficient, cost-effective care management process by determining the patientâs medical necessity and financial liability, and by developing, implementing and monitoring individual patient plans of care. In collaboration with the patient/family, physicians and interdisciplinary team, the case manager ensures patient progression through the continuum in an efficient and cost-effective manner. The case manager is responsible for identifying, initiating and managing optimal patient flow/throughput to enhance continuity of care, smooth and safe transitions, patient satisfaction, patient safety and length of stay management. The RN Case Manager assists with communicating these plans to patients, families, and to the Interdisciplinary Team to ensure quality patient care throughout the healthcare continuum and compliance with all policies and procedures.

Responsibilities ⢠You will organize patient care by prioritizing, delegating, delivering and evaluating on an ongoing basis⢠You will utilize the knowledge base of nursing education and experience to deliver nursing care using the nursing process according to hospital policies, procedures and protocols⢠Collaborate with the nursing team to facilitate overall improvement in quality of care, patient experience and cost reduction⢠Foster a learning environment for patients/families, nursing staff and members of the health care team⢠Advocate for ethical and holistic care promoting the autonomy, dignity, and health of patients from diverse ethnic and social backgrounds Requirements ⢠Bachelor's degree in Nursing, or obtained within five (5) years from the initial date of hire; RN with 15+ years of service or internal candidates hired before January 1, 2021 may be considered in satisfaction of this requirement in lieu of an education agreement⢠Advanced degree in Nursing (MSN, PhD, DNP, ARNP) preferred⢠Registered Nurse license (RN) in Washington State or multi-state licensure endorsement (MSL)⢠Current BLS Certification required⢠ACLS or NALS or PALS or TNCC or NRP certification, as required by facility or department⢠Certification in specialty area preferred⢠One (1) year of experience in acute care setting preferred

We provide a comprehensive benefits package, including competitive salary, medical, dental and retirement benefits and paid time off.
