Title: Resource Center Case Manager

FLSA Status: Hourly, Full-Time, Non-Exempt

Reports to: Resource Center Program Manager


Job Description

The Case Manager at the Rod’s House Resource Center helps support a wide-spectrum of needs for the young people experiencing homelessness who access services. Rod’s House Case Managers are passionate about working with young people, model positive behavior each day, are flexible in an often rapidly changing environment, and are youth-focused in all interactions with young people. Case Managers focus on exiting a young person out of homelessness and into safe and stable housing through emotional, employment, education, and housing supports. Case Managers, first and foremost, prioritize being a valuable team member, but also exude leadership skills while working with their colleagues.


Duties and Responsibilities

  • Establish positive relationships with youth and young adults

  • Conduct intake interviews for new youth and young adults (ages 13-24), screening for program eligibility

  • Develop individual case plans incorporating young people and Case Manager responsibilities

  • While accessing the Resource Center; provide both emergent and support services that address the young person’s unique needs

  • Collaborate with Behavioral Health Specialist to ensure young peoples’ needs are accounted for

  • Work with Resource Center staff to provide youth-centered skills-building and support services.

  • Determine and maintain regularly scheduled case management meetings with each young person

  • Treat all youth and young adults in a trauma-informed, strengths-based, culturally sensitive manner

  • Maintain accurate and quality client documentation in HMIS, Infonet, BFET, and any other database Rod’s House uses

  • Submit End of Month reports to supervisor

  • Ensure standard operating procedures are clearing defined, updated, and followed

  • Regularly assess policies, procedures and processes and recommend improvements

  • Participate in program, department and agency meetings and trainings

  • Participate in local and state interagency work groups, as needed

  • Proven ability to create a system or process when one does not exist

  • Complete all duties as assigned and other duties as required.


Required or Preferred Education and/or Experience

  • One of the following Required:

    • A bachelor’s degree in social services or a closely related field from an accredited school and one year of experience working with youth or young adults OR;

    • A high school diploma or GED and two years of successful full-time experience in a relevant field working with children or youth; supervisory abilities that promote effective staff performance; and relevant experience, training, and demonstrated skills in each area that they will be managing or supervising.

  • Bilingual (Spanish English) strongly preferred

  • Lived experience relevant to the population served (preferred).

Work Conditions and Physical Requirements

This position requires daily interactions with youth experiencing homelessness, community members, and other social service providers. Physical requirements for the position include frequent walking, hearing, seeing, speaking, stooping, kneeling, crouching, reaching, grasping, climbing, and repetitive motions. This position could include exerting up to 50 pounds of force to lift, carry, pull, or move objects. Ability to safely drive a company vehicle is strongly preferred.
