Scope of the Position: We are seeking a dedicated and organized Patient Referral Coordinator to join our team. In this role, you will play a crucial part in facilitating the smooth transition of patients from the acute care setting to aftercare facilities, agencies, or community programs. Collaborating closely with the multidisciplinary team, payers, and community providers, you will ensure that patients receive the appropriate care and support they need after discharge.

Key Responsibilities:

Coordinate the transition of patients from the hospital to aftercare facilities, community programs, or agencies. Provide information and assistance to both internal and external customers regarding aftercare programs and facilities. Manage and organize all necessary paperwork and discharge documents related to patient transitions. Communicate with insurance payers and community providers to facilitate the review and approval process for aftercare placement. Maintain accurate records, reports, and statistics related to aftercare placement activities.

Requirements:

High School Diploma or equivalent. A minimum of three years of experience in a hospital setting or a related health field. Experience in continuing care or home care is preferred.

Skills and Competencies:

Strong organizational skills with the ability to manage multiple tasks simultaneously. Excellent communication and interpersonal skills to effectively collaborate with patients, families, healthcare providers, and external agencies. Attention to detail in handling paperwork and documentation. Proficiency in using healthcare-related software and tools. A compassionate and patient-centered approach to coordinating care transitions.
