As aHospiceMedical Social Worker, Your New Career Will Include:
Aligned Vision: Ensure all activities align with the vision of Elara Caring and contribute to the success of the Home Health team.
Holistic Patient Assessment: Identify psychosocial, financial, and environmental needs through comprehensive patient assessments.
Effective Communication: Share significant findings, problems, and changes in patient condition with the appropriate team members.
Safety Advocacy: Report unsafe conditions and implement safety plans using available resources for patient well-being.
Plan of Care Collaboration: Participate in developing and implementing the Plan of Care to ensure quality and continuity of care.
Skilled Interventions: Deliver skilled services and interventions according to accepted standards and physician's orders.
Patient and Family Support: Counsel, instruct, and involve patients and families in following the Plan of Care and meeting theirsocial work-related needs.
What is Required?
Master’s degree or Doctoral degree insocial workfrom asocial workfrom a school ofsocial workaccredited by the Council ofSocial WorkEducation
Current state license as a Social Worker
1 year ofsocial workexperience in ahealthcaresetting
Social workskills as defined as generallysocial workstandards
50% travel required
Valid driver’s license and insurance and reliable transportation to perform job tasks
You will report to the Clinical Team Manager.
