The Admissions/Intake Charge Nurse for Hospice of Lenawee supports the mission of Hospice of Lenawee through overseeing all aspects of a patient’s admission.

Essential Job Responsibilities:

Communicates Hospice services to prospective patients, families and referral sources.
Gathers initial financial information and communicates with billing representatives.
Works with Clinical Leadership to ensure smooth admission processes.
Enters patient related data into EMR.
Coordinates hospice services with referral sources and discharge planners.
Develops and maintains relationships with area physicians, nursing homes, hospitals, etc. and their staff.
Monitors state, federal and accreditation regulations and standards and assists in educating staff in changes in the regulations.
Reviews current and ongoing quality assurance and quality improvement changes to assure regulatory compliance.
Audits clinical records for compliance with Hospice of Lenawee policies and procedures.
Assists in the review of policies and procedures and forms and protocols.
Participates in ongoing QAPI program.
Responsible for learning and utilizing technology made available by the agency to increase efficiency, communication, and performance of duties.
Be knowledgeable of and follow policies and procedures set by Hospice of Lenawee.
Holds all patient information in strictest confidence and discloses information and data only to persons authorized by Hospice of Lenawee.
Performs other duties and responsibilities as determined appropriate by supervisory staff and/or management.
Demonstrates willingness and flexibility to work different and/or extended shifts when needed.
Presents appropriate professional appearance in accordance with Hospice of Lenawee policies.
Attends staff meetings and completes all mandatory training.
Complies with attendance policy and is physically at assigned location to fulfill job responsibilities and functions.
Assists in conducting meetings/in-services.
Communicates timely, clear, accurate and easy-to-use information to staff, patients, families, referral sources and vendors.
Provides nursing care and supports Home Care and Hospice Home staff when needed.
Performs other duties and responsibilities as determined appropriate by supervisory staff and/or management.
Participates in weekend and Holiday rotation and may be required to work evening shifts.
Hospice of Lenawee direct care staff may have exposure to infectious diseases. Must comply with standard precautions to prevent the spread of infectious disease.

Qualifications:

Demonstrates ability to function as a team member, collaborate with coworkers on different shifts, positions, and departments to promote teamwork and inclusion.
Demonstrates interest, compassion, and empathy for the terminally ill and is comfortable with issues of death and dying.
Is self-aware and asks for help when the need arises.
Demonstrates ability to read, write, and follow direction and instruction.
Demonstrates ability to prioritize and manage time effectively.

Education/Credentials:

Graduate of approved school of nursing.
Registered Nurse with a valid Michigan license.
Valid driver license and vehicle available with current auto insurance.

Experience:

Two years nursing experience, required.
Prior experience in a health care environment, home care or hospice preferred.

Position Status/Classification: Non-exempt, Regular, Hourly, Patient Care
