The Nurse Coordinator is a Registered Nurse, reporting to Nursing leadership within the Pediatric Treatment Centers and under the direction and supervision of the Attending Physician for the specific specialty or site. The primary role of the nurse coordinator is to ensure seamless coordination of patient care across the continuum. This includes clinical practice, care coordination, education, quality improvement, with significant latitude for exercising professional judgement. The Nurse Coordinator is an integral member of the multidisciplinary team providing developmentally appropriate care to patients and families within all Pediatric Treatment Centers. The Nurse Coordinator acts as liaison between patient, family, provider, clinical staff, pharmacy, and scheduling and registration personnel to ensure safe and efficient patient flow.

Responsibilities

1. Professional Practice: ;The Pediatric Nurse Coordinator is the lead member of the health care team coordinating all aspects of patient care for the specific specialty. 2. Education ;Provides patient and family education on appropriate medical condition, including signs, symptoms , treatment regimens, side effects, outcomes expected, and potential adverse reactions or outcomes. Including manangement of condition, promoting optimal level of health. 3. Multi-Disciplinary Collaboration ;Functions as a key member of the multi-disciplinary team coordinating all aspects of patient care across the continuum 4. Quality Improvement ;Participates in data collection related to clinical and operational efficiencies.
Qualifications

EDUCATION

Registered Nurse , Bachelor's Degree in Nursing preferred, Connecticut RN license.

EXPERIENCE

2-3 years' experience in nursing and have knowledge of infusion therapy. Experience in ambulatory setting preferred.

LICENSURE

Licensed as a Registered Nurse from the State of Connecticut. APHON certification preferred not required.

SPECIAL SKILLS

Excellent interpersonal and organizational skills. Ability to assert oneself, advocate for patients, and work independently with minimal supervision. Ability to manage multiple priorities. Computer experience and knowledge of word processing and data entry systems. Must be able to communicate efffectively with all members of the healthcare team, patients and families. Must value and execute "patient -centered model of care."

PHYSICAL DEMAND

Must be flexible to meet the demands of the specialty, outpatient clinic session, specialty and patient/family needs. May need to travel to other sites for clinics.


Additional Information

2-3 year's experience in Nursing and have knowledge in infusion therapy. Experience in Ambulatory setting preferred.
Position is co-terminus with fundng and fully funded by our grants and contracts
