Post-Release Case Manager

Your role will involve:
- Facilitating excellent participant programming:
- Conducting impactful 1:1 and group sessions, developing a comprehensive understanding of every participant on caseload and ensuring treatment goals are met
- Facilitating group sessions focused on life skills such as money management, healthy sexuality, hygiene, self-care, and refusal skills
- Upholding the quality and fidelity of Concordance Re-entry Model curriculum when delivering 1:1 sessions and groups
- Delivering participant Reflections when necessary, in a timely and trauma-informed manner
- Connecting participants to community resources
- Communicating with multi-disciplinary care team:
- Documenting sessions (for individual and groups), including noteworthy participant interactions in electronic record system
- Documenting Reflections using standard forms
- Working collaboratively with the Community and Life Skills team, career coach, and therapist assigned to each participant, providing input on progress to ensure consistency in instruction/learning pre-to-post release and alignment with participants’ goals
- Participating in internal meetings (Community and Life Skills team meetings, All Team meetings, 1:1 supervisor meetings, and training sessions)
Skills, Knowledge, and ExpertiseThe right candidate will come with:

- Excellent interpersonal skills as evidenced by compassion, energy, and ability to build rapport
- High quality verbal and written communication skills
- Experience with documenting participant interactions in case notes
- Familiarity with and endorsement of 12-step programming
- Bachelor’s degree in social service related field (social work, psychology, counseling, criminal justice, sociology, or similar)
- 1-3 years of experience implementing case management services and documenting interactions
- Prior experience with justice-involved adults or other vulnerable populations
- CADC certificate, preferred
- CPR / Adult First Aid certification preferred
