Phoenix Hospice In-patient Case Managers provide compassionate care to clients and their families dealing with a terminal diagnosis in the hospital setting. The philosophy of hospice focuses on the quality of life rather than the quantity. Hospice is a multidisciplinary approach that encompasses the physical, but also the spiritual and psychosocial needs of the patient and their family. The ideal candidate for this position is an RN with strong clinical skills in clinical assessment, diagnostic interpretation, education, organization and is able to multitask in what can be a fast-paced environment. Case Managers work alongside the hospitals interdisciplinary team as well as the hospice team to develop and implement a plan of care that is patient/family centered in the line with the hospice philosophy.
