Join our team as a Weekend Registered Nurse (RN) at our PACE center, covering care for participants in the Topeka, Lawrence, Kansas City, and Emporia areas.

Summary:
This full-time on-call position from Saturday 8am to Monday 8am ensures two days of work with full-time pay, even if not called in.

Midland Care's Program of All-inclusive Care for the Elderly (PACE) is a not-for-profit health care program that helps people at risk for needing nursing home care who meet a level of care requirement stay in their own homes and communities.

Employment Status: Full-time | on-call | Saturday 8am - Monday 8am (PAID even if not called in)

$3,000 Sign-On Bonus **

Pay starting at $33+/hour - pay is based on experience and requirements

Responsibilities
  • Triage and prioritize patient calls, visits, and comprehensive needs.
  • Assessment of patient needs.
  • Planning care, treatment and services which are individualized and appropriate to the patient's needs, strengths, limitations, and goals.
  • Providing the care, treatment, and services according to the plan for care, treatment, and services.
  • Evaluation of care, treatment, and services, with revision to plan of care as needed to meet patient's needs based on assessment, evaluation, and patient goals.
  • Assesses admission eligibility, coordinates, and completes admission timely to ensure successful implementation of hospice plan of care.
  • Assesses discharge and/or transfer needs and facilitates the process to ensure continuity of care, treatment and service is maintained.

Qualifications

Required: Current unrestricted State of Kansas License to practice as a Registered Nurse. Availability of reliable transportation is required. Knowledge of professional nursing theory, standards of nursing practice, scope of practice and relevant statutes and regulations. Ability to prioritize work, meet deadlines, handle multiple projects, pay strong attention to detail and accuracy and work under pressure in a cooperative work environment. Must be self-directed, motivated and flexible. Must possess a valid Kansas Driver's License, in force automobile liability insurance and maintain a Motor Vehicle Record (MVR) that meets organization's insurability standard as defined by Midland policy. Availability of reliable transportation is required for this position.

Preferred: Home Health, PACE, or Hospice experience.
