Essential Duties
  • Lead patient service lines and therapeutic programs to ensure optimal medication therapy.
  • Collaborate with interdisciplinary teams to provide comprehensive pharmaceutical care aligned with evidence-based best practices.
  • Develop policies and procedures to enhance drug utilization and patient safety.
  • Deliver direct patient care and clinical practice, including decentralized and service-based programs.
  • Identify and implement interventions to improve quality and safety, making evidence-based medication recommendations.
  • Provide discharge medication review, reconciliation, and counseling to promote medication adherence.
  • Maintain proficiency in electronic health record systems and medication ordering systems.
  • Communicate effectively with healthcare providers and caregivers to ensure seamless patient care.
  • Participate in pharmacy operations and medication dispensing, ensuring accurate and timely medication therapy.
  • Facilitate specialty medication procurement, ordering, and dispensing procedures.
  • Identify and implement improvements in the medication-use system to advance patient safety and therapeutic outcomes.

Qualifications
  • Doctor of Pharmacy degree from an ACPE accredited College of Pharmacy.
  • Completion of ASHP accredited PGY-1 Pharmacy Residency.
  • Two years of clinical pharmacist experience required if residency is not completed.
  • Strong interpersonal, organizational, and decision-making skills.
  • Excellent verbal and written communication skills.
  • Ability to work independently and as part of a team, with flexibility in approach to time management.
