Description and Responsibilities This position will support The Bridge Counseling Center and the Integrated Dual Disorder Treatment program, working 30 hours in the counseling center and 10 hours with the Integrated Dual Disorder Treatment team. The Bridge Counseling Center serves individuals in the community as well as individuals who are served through other Open Sky programs in an outpatient capacity. We offer evidence-based practices, that include medication management and cognitive-behavioral treatments to support individuals with various clinical presentations. The Integrated Dual Disorder Treatment program is an evidence-based, wrap-around service model for individuals with a dual-diagnosis of a mental health and substance use disorder. The program works with individuals by providing individualized, community-based support from a highly integrated, multi-disciplinary team including a clinician, navigators, recovery coaching, supervisory staff, nursing, and psychiatry. Individuals are provided support through person-centered treatment plans in the settings where they are most comfortable, including the home or other community locations, with the goal of empowering individuals to explore the various pathways to recovery for both mental health and substance use. The LPN will support the operations of the Bridge Counseling Center and Rapid Response Hub to assure timely refills are provided and all authorizations are completed within identified time frames. They will also monitor individuals served to address and mitigate any adverse physiological or psychiatric effects from changes in medication administered while receiving services. Other Key Responsibilities: + Ensure that both programs are in compliance with medication administration policies, and all other related DPH, licensing and/or funder requirements. + Collaborate with the staff team to complete a thorough physical assessment of health needs and provide referrals to appropriate community physicians/health care providers as needed. + Provide training and consultation to programs regarding medical issues, procedures, techniques, and health & wellness practices. + Document all individual encounters and contacts made on behalf of individuals/families and development of Nursing/Health Care Plans and nursing summaries. Ensure effective implementation of Nursing/Health Care Plans through staff training, evaluation, and modification when needed. Qualifications + Valid LPN in Massachusetts, required. + Excellent assessment, documentation, and verbal communication skills required. + Valid driver’s license and acceptable driving history, required. + Previous experience with adults with mental health and substance use disorders, stronly preferred.
