Thriving community health center is seeking a highly motivated, energetic, and dynamic pharmacist to provide safe and effective patient care at Rural Health Group's Stovall, NC Pharmacy. The successful candidate will have exceptional communication and organizational skills in addition to strong knowledge of legal and regulatory standards. This pharmacist will perform the clinical and dispensing services associated with the patient care needs.

The responsibilities of the Pharmacist include but are not limited to the following:

Accept responsibilities of pharmacy manager per NC BOP rules and regulations

Coordinate and provide pharmaceutical care to patients.

Coordinate and optimally dispense medications to patients.

Provide drug information to patients and other health care providers.

Direct workflow, pharmacy technicians, students, and volunteers present during that shift.

Ensure compliance with legal and regulatory standards.

Collect, analyze, and report data required to support patient care activities by optimizing workflow.

Collect, analyze, and report data required to support efforts of pharmacy administration.

Prevent, detect, and resolve medication-related problems and to make appropriate evidence-based, patient-centered medication therapy recommendations as part of the interdisciplinary team.

Determine and resolve any medication therapy problems including drugs without indications, medical conditions for which no therapy is prescribed, therapy inappropriate for medical condition, incomplete immunization record, inappropriate dose, dosage form, duration, schedule, route of administration, method of administration, therapeutic duplication, medication allergies, and clinically significant drug-drug, drug-disease, drug-nutrient, or drug-laboratory test interactions.

Effectively verify patient medication orders for appropriateness and accuracy

Anticoagulation management

Diabetes care education

Provide specialized pharmaceutical services to improve drug usage and therapeutic outcomes including, but not limited to, advising providers on issues concerning drug therapy, the inherent toxicity of drugs and their side effects, as well as assisting in the prescription of appropriate doses.

Coordinate the activities of assigned pharmacy technicians engaged in the preparation, labeling and delivery of medications and other pharmaceutical supplies in accordance with providers' prescriptions; ensure compliance with federal, state and local laws and regulations pertaining to the dispensing of drugs and controlled drugs and the maintenance of required records.

Conduct and evaluate medication histories, assess compliance and suggest modifications to achieve desired outcomes; instruct patients in the proper use of prescribed drugs and participate with providers in pharmacy and therapeutics reviews to evaluate patient progress.

Individualize medication regimens using sound principles, accounting for pharmacodynamic and pharmacokinetic variations in drug absorption, distribution, metabolism and elimination with responsibility for establishing and continually improving the delivery of Pharmaceutical Care to patients within areas of direct responsibility and assisting others in the department with the same.

REQUIREMENTS

Education/Training/Experience: Doctor of Pharmacy Degree

OR

BS Pharmacy Degree with two years of clinical experience or completion of an accredited clinical residency program or applicable professional certifications (Immunization, MTM, AE-C, Diabetes, Anticoagulation, Ambulatory Care as examples) and a demonstrated ability to achieve positive patient outcomes in a similar environment

North Carolina Pharmacist License required

Excellent organizational, telephone and communication skills

Excellent leadership and teamwork skills

Ability to multi-task and work efficiently in a potentially stressful environment

Pleasant, professional approach; must be able to work well with the public and provide excellent internal and external customer service

Ability to establish and maintain positive, effective, professional relationships with patients, providers, subordinates, and superiors

Possess knowledge of modern office equipment and Microsoft Office software programs

Computer skills; must be able to quickly learn to use eClinicalWorks electronic health record system, pharmacy dispensing system, use email system regularly, etc.

Bilingual (English/Spanish) is a plus, but not required
