Phoenix, United States Southwest Network Full time
$5,000 Sign-on bonus available

The ACT Rehabilitation Specialist plays an important role on the Assertive Community Treatment team by helping the members they serve obtain and maintain involvement in employment and/or community-based activities. You'll provide vocational rehabilitation services, situational assessments, and support services to our most vulnerable members and serve as the subject matter expert for the team regarding rehabilitation- and vocational-related services. You will develop relationships with community employers, providers, or other organizations to foster an increase in employment and meaningful community activity opportunities for members.

The ACT model is a service delivery model, not a case management program. You will provide direct services to members and develop plans to help them reach their goals, and review each member's progress daily. You will work with the ACT clinical team to engage, educate, communicate, and coordinate care with each member, their family, behavioral, medical and dental health care providers, community resources, and others to ensure that all services prescribed in each member's individualized service plan are implemented. You must be passionate about pursuing best practice outcomes for members in all areas of their lives, including continuing education, employment, community tenure, social network, independent living status, and decreased substance use.

A complete job description is available upon request.

REQUIREMENTS

To be eligible for this position, you must possess a bachelor's degree and one year full-time related work experience, or an associate degree and at least two years related work experience. Certified Psychiatric Rehabilitation Practitioner (CPRP) preferred. A minimum of one year experience working with job development and/or employment services and resources and working with seriously mentally ill adults is required. Prior Assertive Community Treatment (ACT) or equivalent experience is preferred.

Thorough and working knowledge of psychiatric and co-occurring disorders is imperative. Knowledge of DSM-5 codes and descriptions, psychotropic medications, and crisis intervention required. Knowledge of community based behavioral health services preferred.

Must be able to maintain excellent member and family relationships and provide excellent customer service. Requires effective interpersonal, written and oral communication skills. The use of sound judgment and excellent time management skills are essential. Must be able to follow direction, identify priorities and stay organized. Must be competent in the use of electronic medical records software, as well as in the use of a computer, printer, copier, and telephone. Preferred typing skills of 25 to 40 WPM. Must be highly proficient in the use of Microsoft Office software, including Outlook, Word and Excel. Must be able to effectively use the internet and various web browsers.

Must maintain current CPR, First Aid, and Therapeutic Options certifications, as well as self administration of medication certification. Must have and maintain a valid Arizona driver's license.

Only candidates 21 years of age or older will be considered.

COMPENSATION

This position is offered at a fair, competitive salary. A good benefits package is a valuable part of your total compensation, and we've got a great one: medical, dental and vision insurance; paid sick, vacation and holiday time; tuition reimbursement; retirement plan with match; disability insurance; life and AD&D insurance; flexible spending and health savings accounts; an employee assistance program; employee discounts and more. When you consider everything-the salary, the benefits, the ongoing training and support we offer, and the impact you'll have on the lives of others-it could be the right opportunity for you.

Any offer of employment is contingent upon verification of education, employment, driving record and auto insurance, and completion of a criminal background check and drug testing.

Southwest Network is an equal opportunity employer.

ABOUT OUR ADULT SERVICES DIVISION

For adults with serious mental illness, recovery and success in life depends on good mental and physical health, as well as having satisfying relationships, safe housing, purpose in life, a sense of belonging, and hope for the future. We help people define their personal goals for recovery and manage their symptoms so they can achieve those goals. With our integrated health services, care and support, they discover strengths they didn't know they had and are able to build futures filled with hope, meaning, and wellness.

Our Assertive Community Treatment (ACT) program focuses on providing intense, frequent, face-to-face services to members with persistent and severe mental illness and/or substance use disorders. Most services are provided in the community, when and where our members need them. ACT services reduce crises and helps members learn skills that are needed to integrate into the community and live independently without supports. This evidence-based practice helps improve outcomes for people who are most at-risk of homelessness, psychiatric hospitalization, and institutional recidivism.
