The MGH Cellular Therapy program seeks a dynamic and experienced Patient Navigator & Community Liaison to provide and oversee case management services for patients receiving primary and specialty care within the Cell Therapy Clinic. The mission of the Mass General Cellular Immunotherapy Program is to invent, develop, administer, and understand engineered immune effector cells. We seek a candidate dedicated to supporting all these services and the patients we serve.

This incumbent should have the flexibility to provide a range of services, including case management, program development, administrative coordination of appointments and treatment, and ongoing support to patients needing acute care services.

This is a full-time, 40/hrs per week, Monday - Friday hybrid position in an outpatient clinical setting at the MGH Boston Campus.

PRINCIPAL DUTIES AND RESPONSIBILITIES:

Patient Care Services Navigation Duties

  • Working under the supervision of the Administrative Manager, the incumbent is responsible for attending all clinics with a multidisciplinary team, addressing relevant patient care issues, and advocating for care services.
  • The incumbent will oversee the scheduling function for all appointments needed by patients.
  • Participate and support the intake process of new patients.
  • Provide case management services, including assessment, scheduling, and records collection.
  • Documents timely (within 72 hours) and relevant information in the patient's electronic medical record. Maintain expected timely expectations set forth by the Medical Director and Administrative Manager
In addition to the services and duties listed above, the Patient Navigator will be expected to meet the following needs:
  • Involved in the welcoming of new patients to the program by meeting with them either in person, virtually, or over the phone
  • Willing to assist with processing internal and external referrals for care services related to their diagnosis, including working with insurance carriers to secure coverage for care and ensuring that outgoing referral paperwork is appropriate and complete.
  • Willing to assist with outgoing referrals to appropriate services when appropriate.
  • Part of the review of incoming referral documentation for completeness and appropriateness
  • An active participant in interdisciplinary treatment planning team meetings and other staff development activities
  • A positive collaborative member of the team, referral sources, and critical community providers
  • Able to utilize the electronic health record (EHR) system to ensure proper and timely completion of all required clinical documentation, program statistics, and outcome data.
Administrative
  • Collaborate with administrators and clinical staff on continuous improvement of the MGH Cellular Therapy program and standards of practice in health care delivery.
  • Participate in ongoing quality assurance programs.
  • Attend staff meetings.
  • Identify areas of operational improvement
  • Manage implementation of new processes in clinic
SKILLS & COMPETENCIES REQUIRED:
  • Computer literacy in Word, PowerPoint, and Excel
  • Able to utilize technologies such as Epic EMR, Zoom, and other resources to make sure patient care is well documented; training in Epic EMR will be provided for those who have not used this interface previously.
  • Experience working with patients from culturally diverse backgrounds, and the ability to demonstrate cultural competence and sensitivity
  • Knowledge and abilities essential to the successful performance of the duties assigned to the position
  • Demonstrates resourcefulness, effective written and oral communication, diplomacy, and organizational and analytic skills.
  • Self-directed, pragmatic, and creative in problem-solving, systems planning, and patient care management in a high-volume work environment
  • Ability to work effectively and collaboratively with interdisciplinary teams.
  • Demonstrates case management skills and experience in the community health care delivery system.
  • Exceptional communication and presentation skills relating to the frail elderly, functionally impaired adults, their support systems, and teams of health professionals.
Qualifications

QUALIFICATIONS
  • Bachelors Degree required
  • five years of patient care services experience required
  • Experience in outpatient and hospital settings
Preferred:
  • Administrative services/case management experience highly desired
