JOB SUMMARY Provides leadership, resource, and oversight of the care management team at Windham Hospital, Collaborates with the care management team in the East Region and at the system level. Works with leadership at Windham Hospital to provide cohesive program management and with the Regional Director of Care Management. Is responsible for the daily operation as well as specialty programs which encompass transition planning, social work, clinical resource and utilization management. Comprehends and embraces the changing healthcare landscape including care delivery redesign and alternative payment models and effectively leads departments through this transformational change. Embodies innovative thinking specifically around the continuum of care, identifying opportunities for constituent engagement in the pre and post-acute settings.Qualifications Education: Bachelor's degree in Social Work (BSW) or Nursing (BSN) required. MSW/MSN or Master's in relevant healthcare field preferred.Experience: 3 years of health care experience; 3-5 years managerial experience required. 5 years of health care experience; 5-7 years managerial experience preferred.Licensure, Certification, Registration: Nurses require State of CT RN licensure. CT licensure and certification in relevant field preferred. Knowledge, Skills and Ability Requirements Ability to read and understand oral and written instructions, and effectively communicate information. Basic computer skills (including opening/reading email, basic internet use) Must possess expert level of knowledge in departmental field of concentration. Ability to effectively monitor and manage the effectiveness of departmental function and programs. Demonstrated ability to develop short and long term operational and strategic plans. Broad understanding of business strategies and implementation processes. Demonstrated leadership abilities. Ability to communicate technical or complex information and present effective and persuasive discussions with all levels of the organization. Also has the ability to represent WMCH to external agencies and organizations. Ability to manage time and meet deadlines in a stressful work environment. Ability to handle multiple priorities.
