Rehabilitation Coordinator

Job Summary:

  • Coordinate and provide services to promote rehabilitation, recovery, and resiliency
  • Identify and address strengths and barriers to individual/family recovery and independent functioning in the community
  • Support individuals and families in crisis situations with personalized interventions to enhance decision-making abilities
  • Support at-risk children, adults with severe mental illness, and individuals with substance abuse and co-occurring disorders

Responsibilities:

  • Assist patients with access to medical services, community resources, and financial assistance programs
  • Collaborate with medical providers, multidisciplinary teams, and community resources to help patients achieve treatment goals
  • Facilitate life skills development, including activities of daily living, interpersonal coping, socialization, and community functioning
  • Plan for professional growth and development related to individual goals for self-assessment, evaluation, and feedback
  • Provide wellness resources and mentorship to promote recovery and resiliency, including symptom monitoring and management, medication education, and self-motivational skills
  • Assist in developing and coordinating individual service plans, including recovery/resiliency management, crisis management, and behavioral healthcare advanced directives
  • Coordinate and monitor client activities and service usage, document client encounters, and maintain comprehensive client files
  • Assist clients in obtaining necessary rehabilitative, medical, and housing services, and mentoring to those involved in the Judicial System
  • Encourage development of natural support systems in the workplace, school, family, and community
  • Conduct ongoing assessments to determine effectiveness of services accessed and intervene in crisis situations to facilitate use of individual management plans and alternatives to emergency hospital services
  • Maintain applicable program and client service statistics

Requirements:

  • Bachelor's degree in related discipline
  • 1 year directly related experience
  • CPR for Healthcare/BLS Certification required
  • Certified Psychosocial Rehabilitation Practitioner or Licensed Alcohol and Drug Abuse Counselor preferred
  • Ability to perform light work, including walking, standing, pushing, and pulling
  • Ability to work in physical environments with minor hazards
