Job Type Full-timeDescriptionWORK SUMMARY: The Case Manager will be the primary contact for participants starting at outreach and will facilitate connections to other workforce and supportive services utilizing a strong community-based network of resources. Case Manager will ensure that participants are served in a culturally competent, trauma-informed manner to help them define their career aspirations and co-create a plan to achieve their goals. Also, Case Manager should be an active problem solver in partnership with participants to address their long-term goals.ESSENTIAL FUNCTIONS:Engagement & Recruitment: Communicate with individuals from a wide variety of backgrounds and experiences. Use technology and social media platforms to engage participants from the target populations.Intake screening & assessment for case planning: Responsible for administering an approved intake screening and assessment(s). The screening and assessment tools are used to develop the participant's IEP (Individual Employment Plan), assess areas of strength, and identify priorities based on their chosen career pathway and need for resources.Career Coaching: With direction from the participant, the CM will connect and coach their clients to services. CM will also help participants understand the opportunities available to them to advance their career goals and will track referrals to job interviews and further training, education, and skill development opportunities and will facilitate the Job Readiness Classes.Managing Accounts: Update and maintain client files and databases (ECM) as required by program contracts. Analyze client needs and provide effective solutions. Proactively addresses problems and ensures timely resolution for customer concerns. Reacts positively to change.Wrap-around services and supports: CM will work with participants to ensure that they have access to an array of services as needed Career Navigator will provide services directly of make referrals for services, including providing transportation to and from appointments as needed and/or transportation support (CTA/PACE bus cards). Lastly, the CM will collect data and report on participant progress, challenges and successes to identify potential service delivery gaps. RequirementsEDUCATION, EXPERIENCE AND SKILLS REQUIREDMinimum Associate degree or higher in related field. Bachelor's degree preferred.Experience working with substance abuse population and individuals with multiple barriers to employment.Licenses/Certifications: A valid driver's license is required.Language skills: Excellent written and verbal communication skills.Computer Skills: Proficiency in Microsoft Office, Internet Explorer, Internet search Engines, EXCEL spreadsheets, Outlook/Exchange and other software routinely used by Haymarket Center.
