Position Case Manager – APIVPS & Connective Services

Salary $25.00- $28.00 per hour plus benefits

Reports to Program Director

Status Full Time (1.0 FTE), Non-Exempt

POSITION DESCRIPTION The CYC Case Manager will work under two programs – Asian Pacific Islander Violence Prevention Services (APIVPS) and the Connective Services, which are two programs that offer case management services, SEL support groups, and enrichment activities for in-risk/ at-risk API youth as well as youth involved in the Juvenile Justice System (ages 10-24 years old) and their families. These programs aim to reconnect youth with caring adults and peers by providing financial, social, and recreational resources in addition to building individual strengths and promoting youth and family communication.

Under the supervision of the Program Director, the CYC Case Manager is responsible for providing services to high risk Asian and Pacific Islander youth and their parents in schools, community centers and the Juvenile Justice Center through coordinated intensive individual and family care management services and outpatient mental health counseling when appropriate.

DUTIES AND RESPONSIBILITIES

Conduct assessments to determine needs of the youth and their families.

Attend court hearings if needed and provide advocacy for youth and their families.

Develop individualized care plan and monitor implementation of treatment goals.

Ensure service linkage to the mental health services including assessment and counseling sessions.

Advocate for clients to reduce recidivism contact with juvenile justice system.

Provide individual & family care management and support and referral services.

Facilitate meetings with related parties during the intervention/reintegration processes.

Maintain official contacts to schools, community organizations, and other service providers

Develop cooperative relationships with school personnel, probation officers, parents and families, collaborative agencies and other service providers.

Attend client review meetings, clinical meetings, and agency meetings as required.

Work in conjunction with other CYC program to provide effective, comprehensive services.

Facilitate meetings with related parties during the intervention/reintegration processes.

Participate in trainings, team meetings and community meetings as necessary.

Maintains accurate documentation and statistics on client services, and produces monthly reports for granting agencies.

Support events, activities, and other projects that promote violence prevention and reduction in the Asian and Pacific Islander community.

Other duties as assigned by the Management.

QUALIFICATIONS/REQUIREMENTS

Bachelor’s degree in social services or related areas plus minimum of two years of care management experience.

Experience in conducting interviews and assessment with youth and parents.

Experience working with San Francisco’s diverse communities and neighborhoods.

Experience in conducting support groups.

Knowledge of San Francisco Juvenile Justice System.

Knowledge of community-based agencies serving youth in S.F.

Strong interpersonal skills to work with youth, parents and probation officers.

Excellent verbal and written communication skills.

Strong organizational and collaboration skills.

Able to lift or move at least 25lbs

Willingness to work on evenings, weekends and outdoor activities

Bilingual in English and Chinese (Cantonese and/or Mandarin) highly preferred.

Ability to work with ethnically diverse groups of children, adults, and families

Commitment to taking regular Covid-19 health tests as required by program health and safety guidelines and/or current health directives.
