Case Manager

to oversee the care needs and the In Home Care packages for our clients. The Opportunity

The Case Manager Home Care Packages will be responsible for ensuring that we are providing an efficient professional service delivery of the highest standard. We are looking for someone who is motivated and passionate about aged care and the delivery of high-quality customer care. Key Responsibilities

Provide Case Management support to the clients of Seniors Community Care based on best practice Undertake intake and monitoring within a professional and case management framework Comprehensive assessment of care needs Coordinate delivery and brokerage of services within the package budget Communication with care recipients, family and service providers Build awareness within the community and service providers of Seniors Home Care Packages Program Your experience Tertiary qualification in health industry or related field is desirable Experience in case management within the home care package environment including complex case management Strong understanding of Home Care Package Programs Ability to adhere and maintain compliance with the Aged Care Quality Standards Demonstrated knowledge of community resources Ability to work autonomously as well as working as part of a team We are committed to creating a diverse environment and are proud to be an equal opportunity employer.
