SUMMARY

Position renders professional, skilled nursing services to the patient and their families according to a physician’s plan of care. Instructs and supports the activities of the Home Health Aide as it relates to patient care.

JOB DUTIES

Provides an initial evaluation visit to all patients referred to Hospice for service.

Utilizes the nursing process to meet the nursing and home health needs of patients and families and formulates appropriate nursing care plans to meet these needs on a regular basis.

Initiates, reviews and revises the established patient plan of care as necessary in consultation with the Interdisciplinary Group.

Provides those services, treatments, diagnostic and preventive procedures which require substantial specialized nursing skill to patients as defined by the plan of treatment.

Initiates appropriate preventative and palliative nursing procedures as appropriate for the patient’s care and safety.

Prepares and maintains patient’s records, including clinical and progress notes, and related reports in accordance with Agency policy.

Coordinates services provided to individual patients to assure continuity and comprehensiveness in all levels of Hospice care.

Informs physicians and other personnel of changes in patient’s physical or emotional condition and/or needs taking timely and appropriate action with regard to pain and symptom management.

Reviews all medications taken by patient, identifying all possible ineffective drug therapies, adverse reactions, significant side effects, drug allergies, and contraindicated medications reporting to physician as appropriate.

May frequently attend deaths, communicate with funeral directors, and contact physicians at the time of death.

Demonstrates, teaches, supervises and counsels families and/or other persons in regard to patient care needs and/or other related problems.

Counsels the patient and family in meeting nursing and related health needs.

Attends institutes, workshops, in-services and/or other professional meetings, as required or preferred by supervision, to enhance ones knowledge in Hospice/Palliative care.

Documents ongoing education quarterly-self directed.

Attends and participates in Agency staff and Interdisciplinary group meetings.

Participates in Agency committees relating to patient care and quality improvement.

Supervises and provided written individualized patient care instructions to home Health Aides and volunteers.

Conducts supervisory visits on at least a bi-weekly basis, whether aide is present or not, to assess services provided and needed documentation on forms per agency policy.

Maintains appropriate contact with bereaved families.

Participates in and/or initiates bereavement POC utilizing initial Bereavement Assessment and follow up forms.

Documents ongoing bereavement issues.

Receives referral information for admission services.

Provides “On-Call” and emergency services as assigned.

Participates in volunteer program.

Ability to respond to unusual or difficult patient/family requests.

Ability to explain and/or teach patients procedures necessary to improve/maintain health and safety.

Ability to maintain a sense of propriety in patient relations.

MINIMUM EDUCATION, KNOWLEDGE, SKILLS, AND ABILITIES REQUIRED

Graduate of a hospital, diploma school of nursing, associate degree or BSN program, approved by the State Board of Nurse Examiners.

Licensed to practice as a registered nurse in Pennsylvania.

One year experience in general staff nursing, preferable in an acute care setting, desirable. Home health experience desirable.

Licensed, or access to transportation, driver with current Pennsylvania License and careful driving record.

Auto insurance is required by the State of Pennsylvania and proof of current coverage is required at all times.

Credentialing/Competencies in accordance with current credentialing and/or competency requirements

BLS required.
