•CDL-Commercial License class A or B
•DL-51 DMV medical form (Medical Examiners Certificate)
•DL-45 California Special Driver Certificate
•First Aid Card (if restriction on school bus certificate)
•H-6 DMV Information Request (Driving History valid within 30 days)
•T-01 (original copy) Training Certificate
•T-02 (copy) Instructional Documentation
