Description: Position Summary This is a full time onsite Float Discharge Planning HURC II position Monday to Friday 8am to 430pm with every other Saturday.

Duties & Responsibilities

Facilitates coordination of patient services in assigned patient care unit, or department._x000d_ Collaborates with the patient, family, physicians, RNs, and multidisciplinary team to assure_x000d_appropriate safe, cost effective, timely and efficient services. Utilizes clinical and social work experts_x000d_as needed to ensure delivery of comprehensive patient services._x000d_ Collaborates and communicates with all staff in developing and implementing the discharge planning_x000d_process including: appropriate CLOC, continued stay, patient assessment (for appropriate Clinical_x000d_Level of Care (CLOC), continued stay, planning, implementation, interdisciplinary collaboration and_x000d_ongoing evaluation. Documents risk assessment and discharge planning information in the medical record according to_x000d_department standards. Coordinates and provides discharge planning. Screens patients for high risk discharge planning needs_x000d_and assures collaboration with interdisciplinary team members. Qualifications: Minimum Qualifications

Graduation from an accredited professional nursing program Current licensure as a Registered Professional Nurse with the Illinois Department of Financial and Professional Regulation, Division of Professional Regulation. Five years (60 months) of RN experience. Current AHA BLS/CPR and other required unit specific certifications Effective communication skills (oral and written) that facilitate therapeutic relationships between patients, family, and health care providers Intermediate computer skills with the ability to become proficient in the Electronic Health Record (EHR) with on-the-job training Excellent attendance, interpersonal skills and work history required.
