Schedule: Full Time (40 hours/week) Day shift, may include weekends

Job Description:

Supports the admission process by providing professional nursing assessment information. Interviews potential resident and family; performs medical record reviews to assist in selecting admission and works with the Providence Extended Care (PEC) and Providence Transitional Care Center (PTCC) admission team in preparing new resident equipment needs and room set-up requirements. Coordinates between hospitals, PEC and PTCC, as required. Assists the admission coordinators to assure that all new resident admissions are completed in an appropriate, smooth and timely manner with respect to individual dignity and quality life issues. The responsibilities of the Admission Nurse are centered around the MDS support documentation to justify skilled care and to meet regulatory expectations. Provides support to Nursing Services as directed.

Required Qualifications:

  • Graduate of an accredited School of Nursing.
  • Alaska Registered Nurse License upon hire.

Preferred Qualifications:

  • Bachelor's Degree - Nursing.
  • Certification in Gerontological Nursing upon hire.
  • 3 years - RN clinical experience.
  • Admission and/or long-term care experience.

About the Team

Providence Home and Community Care (HCC) is a service line within Providence with over 8,000 caregivers and 2,000 volunteers serving nearly 200,000 people a year, in their homes, in clinics and in a variety of congregate living ministries across our seven-state footprint.

HCC provides a full range of services and support for people of all ages, especially those facing chronic or life-limiting illnesses. Our core competencies include restorative care, longitudinal care and care through the end of life. Our service lines include assisted living/skilled nursing/rehabilitation, home health, home infusion/pharmacy services, home medical equipment, hospice and palliative care, Program of All-Inclusive Care for the Elderly (PACE), personal home services/private duty care and supportive housing.
