YOUR CONTRIBUTION TO OUR SUCCESS:
A Retention & Re-engagement Specialist contributes to AHF by supporting the Department of Medicine in achieving its retention goals. This role works closely with providers and support staff to recognize patients at risk for falling out of care, works with newly diagnosed clients to encourage participation in their care and identifies clients who have fallen out of care and reengage them.

 

A Retention & Re-engagement Specialist enhances the reputation of AHF by serving as a liaison to the community, as a resource and an additional touch point for the client.
You will be a passionate advocate for our top initiatives.

 

To be successful as a Retention & Re-engagement Specialist it is crucial to ensure that the AHF Core Values and Mission stay top of mind with all that you do. At AHF we are nimble and able to adapt in a dynamic environment to assist in providing the best experience for our clients and workplace for our employees.

AHF has a collaborative organizational structure where staff are accountable to multiple leaders.

 

The Retention & Re-engagement Specialist will work as a part of a close-knit team.
