We provide compassionate, personalized in-home care focused on safety, independence, and quality of life. We're seeking a licensed Registered Nurse (RN) to join our team as a Nurse Manager / RN Clinical Liaison a hybrid clinical and outreach role combining nursing leadership, in-home assessments, and relationship building with clients, families, and referral partners. What You'll Do Conduct in-home assessments and reassessments for new and existing clients (VA and private pay). Develop individualized care plans and ensure all services meet clinical, regulatory, and quality standards. Supervise caregivers performing delegated tasks; ensure compliance with state and VA regulations. Act as a liaison with Veterans Affairs case managers, social workers, and community partners. Conduct intake and sales visits for prospective private-pay clients assess needs, present services, and convert inquiries to admissions. Train and mentor caregivers to ensure skill competency and adherence to agency standards. Oversee timely authorizations and service renewals for VA and other payer programs. Participate in quality improvement, incident review, and staff development initiatives. Travel regularly within the service area to meet with clients, families, and referral sources. Company vehicle available for travel. Who You Are Licensed RN in Wisconsin and Minnesota (or compact license). 2+ years of home health or community nursing experience. Experience with VA programs or veteran clients strongly preferred. Skilled in assessment, documentation, and care planning. Outgoing and professional comfortable meeting clients, families, and referral sources. Organized, self-motivated, and able to work independently. Valid driver's license and reliable vehicle required (travel reimbursed). Passionate about improving the lives of seniors and veterans.
