Job Summary

We are seeking a compassionate and dedicated Care Coordinator to join our team. The ideal candidate will play a vital role in ensuring that our clients receive the highest quality of care. This position involves coordinating care plans, assisting with daily living activities, and providing support to individuals with various needs, including those with developmental disabilities, dementia, and Alzheimer's disease. The Care Coordinator will work closely with families, healthcare providers, and support staff to enhance the quality of life for our residents.

Shift Details

  • Monday - Friday
  • 8:00 a.m. - 5:00 p.m.
  • Will need to cover CNA, PCA, or Med-Tech shifts as needed

Duties

  • Develop and implement individualized care plans tailored to each resident's needs.
  • Assist clients with Activities of Daily Living (ADLs) such as bathing, dressing, meal preparation, and medication administration.
  • Provide direct support in memory care settings and assist individuals with Alzheimer's and dementia.
  • Monitor vital signs and report any changes in health status to the appropriate healthcare professionals.
  • Maintain accurate documentation using EMR systems while ensuring compliance with HIPAA regulations.
  • Facilitate communication between families, caregivers, and healthcare providers to ensure cohesive care delivery.
  • Conduct assessments to identify client needs and develop strategies for effective behavior management.
  • Provide training and supervision for staff on caregiving techniques and conflict management.
  • Assist with cleaning duties to maintain a safe and hygienic environment for residents.
  • Drive clients to appointments or activities as needed.

Qualifications

  • Preferabbly LPN Licensed
  • Previous experience in assisted living, nursing homes, or senior care settings is preferred.
  • Familiarity with working with people with developmental disabilities and autism is a plus.
  • Knowledge of medical terminology, first aid, and patient care practices is essential.
  • Experience in hospice care or home care settings is advantageous.
  • Strong interpersonal skills with the ability to manage conflicts effectively.
  • Ability to perform heavy lifting as required by the role (e.g., using a Hoyer lift).
  • Proficiency in case management principles and developing care plans is desirable.
  • A background in social work or nursing can be beneficial for this position.

Pay: $60, $65,000.00 per year
