General Description of Duties:

The Program Manager is responsible for the growth, development, and day-to-day management and oversight of the assigned Residential location, the supervision of staff within the residential location, and acting as a liaison with other programs and supporting departments representing the Residential Department. Assigned homes will be no more than 4 residential homes that houses up to 5 individuals. Scheduling will be either Sunday- Wednesday or Wednesday- Saturday and as needed. 

Schedule is as follows: 

Sunday 8am-9pm Wednesday- 9am-5pm

Monday 9am-7pm Thursday- 9am-6pm

Tuesday 9am-6pm OR Friday 9am-7pm

Wednesday 9am-5pm Saturday-8am-9pm

Essential Tasks and Responsibilities:

In a manner, consistent with supporting Langton Green, Inc. Mission Statement, the Program Manager shall do the following:

  • Ensure all individuals served are living according to their rights and responsibilities and are free from abuse and neglect.
  • Communicate and interact positively and effectively with each person receiving supports, visitors, family members, peers, and supervisors.
  • Coordinate staffing in all assigned homes under the supervision and approval of the Program Director. Review ongoing staffing needs, and schedule shifts that are vacant or impacted by staff call outs, with appropriate, and trained personnel while ensuring staffing ratios are in place at all times.
  • Implement and maintain an ALU/Group Home review system. The Program Manager will conduct episodic visits (daily, weekly, bi-weekly, etc.) of households, with a particular focus on homes with known concerns, new staff, or a series of unexplained unusual incidents.  
  • Identify additional training needs of staff, or the need to repeat formal trainings based on interactions with Direct Care staff and refer these issues to the Program Director.
  • Review and monitor the implementation of Person-Centered Plans (PCP), Behavior Plans (BP), Planned of Care and PMOF by the: collection of data on goals, daily logs, incidents reports, progress notes, 45-day nursing assessments, all individual specific ancillary paperwork, and a review of medication in maintaining administration records.
  • Assist as part of the Interdisciplinary Team in the development of individual PCPs. This includes assisting in developing planning material, suggesting and developing individual goals, and represent residential services at PCP Meetings.   
  • Perform emergency contact duties for the agency as needed.
  • Document all on-call activity for the assigned homes and maintain a constant upstream of information with the Program Director.
  • Provide on-call supervision to ensure that appropriate staffing is in place at all times to ensure routine operations are running effectively.  
  • Responsible for ensuring proper training is provided to substitute staff when necessary. 
  • Serve as a member of the Management Team by maintaining all agency policies and procedures.   
  • Responsible for the overall supervision of all assigned residential homes to ensure the safety, cleanliness, neatness and appropriateness for individuals receiving services. 
  • Responsible for the recruitment, selection, training and performance management and timely annual reviews of staff.
  • With prior Program Director approval and involvement, implements necessary and appropriate corrective disciplinary procedures, including separation of employment, to ensure acceptable levels of performance and conduct.
  • Ensure and attends regular house meetings, to ensure and communicate efficient, effective methods and practices as well as other issues as they arise.
  • Checks timesheets for accuracy. Approves/denies requests for time off and accurately records usage of such time.
  • Attend all in-services, staffing and other meetings as requested by Program Director.
  • Report all Quality Enhancement (QE) concerns and issues to the Program Director as soon as possible, this includes but is not limited to, any health and safety concerns and allegations.
  • Report significant changes, issues, and incidents regarding people receiving services, and programming to the Program Director.
  • Works effectively and cooperatively with the individual's family, legal guardian, stakeholders, day program staff, nurses, service coordinators to ensure individual is receiving best quality of care.  
  • Oversees that each individual's healthcare needs are met. This includes maintaining up-to-date medical appointments, ordering medication, ensuring that all follow-up appointments and referrals are completed, and all appropriate documentation is obtained for each medical appointment and/or procedure all within a timely manner.
  • Monitor and review all medical documentation while providing oversight to ensure medical goals are achieved through the Nursing Plan of Care. These reviews will concentrate on areas of concern but can also include all other areas subject to review.
  • Monitors household supplies such as groceries and requests for items, and then oversees the purchasing and storage of items.
  • Maintain all policies, procedures, and ethical responsibilities set by the agency.
  • All other duties as assigned.

Supervisory Responsibilities:

Supervises Direct Support Professionals (DSP).

Qualifications:

Knowledge and understanding of the needs of individuals with disabilities is preferred. Must have demonstrated abilities in supervision, budget management, and service delivery. Must be able to demonstrate skills in the following areas: problem solving, project/task management, time management, dependability, good judgment and safe work practices. Current maintenance of all required training's under DDA and CMT guidelines and Langton Green. Flexibility of scheduling to meet Agency and program needs.

Education and/or Experience

The Program Manager will be outlined in Tiers.  

Tier I: Program Manager I- Must have Five to Seven years' experience as a direct care, case management and or coordinator, most of which must be in residential services.

Tier II: Program Manager II- Must have BA/BS Degree preferably in a social science (Human Service and/or Psychology) with One to Five years' experience as a direct care, case management and/or coordinator, most of which must be in residential services.

Tier III: Program Manager III- Must have BA/BS Degree preferably in a social science (Human Service and/or Psychology) with Five to Seven years' experience as a direct care, case management and/or coordinator, most of which must be in residential services.

Fluent in Microsoft Office Suite (Excel, PowerPoint, Outlook and Word).

Language Skills:

Ability to read and interpret documents. Ability to write reports and complete forms or other documents as required. Ability to communicate with others including individuals in Langton Green's programs, family members, medical professionals, stakeholders and co-workers.

Certificates, Licenses, Registrations:

Must have valid Motor Vehicle Class "C" Driver's License and access to reliable transportation.

Maintenance of all required training's under DDA and CMT guidelines and Langton Green's.

Physical Demands:

While performing the tasks of this position, the employee is required to stand, walk, sit, bend, kneel, and use hands to finger, handle or feel, reach with arms and hands, balance, lift, stretch.  

Must be capable of lifting/moving over 50 pounds and the transferring of individuals with or without equipment. Vision abilities required for this position include, near distance, far distance, peripheral vision, color vision, depth perception and the ability to adjust focus.
