Job Title: Admissions Nurse

As an Admissions Nurse, you will play a vital role on the hospice team, ensuring a smooth and efficient admission process for patients. Your expertise will be essential in assessing patient needs, coordinating care, and providing compassionate support to individuals and their families. What you'll do

  • Conduct thorough patient assessments to determine medical history and current health status is hospice appropriate.
  • Collaborate with healthcare professionals to develop individualized care plans tailored to each patient's needs.
  • Manage the admission process by completing necessary documentation and ensuring compliance with regulatory standards.
  • Provide education and support to patients and families regarding treatment options, procedures, and post-admission care.
  • Monitor vital signs and perform initial diagnostic evaluations to identify any urgent medical issues.
  • Coordinate with various departments to facilitate timely access to services such as imaging, laboratory tests, and specialty consultations.
  • Maintain accurate medical records and documentation in compliance with HIPAA regulations.
  • Advocate for patients' needs while working closely with interdisciplinary teams to ensure high-quality care delivery.

Basic qualifications

  • Current nursing license (RN ) in good standing.
  • Strong knowledge of medical terminology and patient care protocols.

Preferred qualifications

  • Experience with Electronic Medical Record (EMR) systems such as Epic or Cerner.
  • Familiarity with ICD-9 and ICD-10 coding for accurate medical documentation.
  • Proficiency medication administration, and basic life support techniques.
  • Experience working with diverse populations, including those with developmental disabilities or chronic conditions.
