**Job Overview – Community Health Worker:

Compensation: $25 – $35/hour

Location: Boston, MA

Schedule: Monday to Friday (Hybrid)**
Atlantic Group is hiring a Community Health Worker in Boston, MA for our client, supporting community-based outreach, care coordination, patient education, and social services navigation for underserved populations. This temporary role engages patients in clinical, home, and community settings while supporting care planning, transitions of care, and connections to essential resources in collaboration with interdisciplinary care teams.

Responsibilities as the Community Health Worker:

  • Patient Outreach & Engagement: Conduct face-to-face and community-based outreach to enroll patients, complete screenings, and provide ongoing follow-up across hospital, clinic, and community settings.
  • Care Coordination: Partner with patients and care teams to set goals, support appointments, medication access, and care transitions, including hospital and emergency department discharges.
  • Community Resources & Advocacy: Connect patients to social services such as housing, medical assistance, SNAP, transportation, utilities, and behavioral health supports through direct assistance and warm handoffs.
  • Health Education & Collaboration: Deliver culturally appropriate health education, document care activities and progress in the EMR, and collaborate closely with interdisciplinary care teams.

Qualifications for the Community Health Worker:

  • Education: Bachelor's degree in Public Health, Social Work, Human Services, or a related field preferred.
  • Certification: Massachusetts CHW certification preferred.
  • Experience: 2+ years of experience in healthcare, public health, or community-based outreach roles, preferably working directly within diverse or underserved communities.
  • Technical Skills: Proficiency in Microsoft Office applications, data entry, data tracking, and electronic medical record systems.
  • Industry Knowledge: Basic understanding of the healthcare system, social determinants of health, care coordination, and community-based resource networks.
  • Skills & Attributes: Compassionate, culturally competent professional with strong communication skills and the ability to work independently and collaboratively in community-based settings.
  • Additional: Valid driver's license with reliable access to a vehicle required.
