Description

Position Description: The Cross Agency Social Worker or Counselor position will be part of a Cross Agency team that provides wrap-around services to students and families by serving as the point of communication and collaborative care with area agencies that support our residents. Through their work, the Cross Agency Social Worker/Counselor will coordinate care from Sauk Prairie School District, the Human Services branch of Sauk, Dane, and Columbia Counties, area hospitals and behavioral health providers, and Sauk, Dane, and Columbia County law enforcement.

Responsibilities for the position include but are not limited to the following:

• Collaborate within the Cross Agency Team and with building student services, teaching and administrative staff.

• Serve as a bridge to Community and County Services for student and family needs identified by school staff, students, families and area agencies.

• This could include:

  • Attend county meetings when applicable.
  • Connect with local hospitals and clinics to serve as a conduit for community outreach and education.
  • Establish and engage in coordinated relationships with county agencies, medical and behavioral health providers, community support agencies, and law enforcement.
  • Engage in home visits with students and their families.
  • Provide strategies, skills, education and support for students and families.
  • Be available to support a student who is experiencing a mental health crisis to provide mental health support. This support is provided during the school year school day and hours established over the summer.
  • Serve as the point of contact to assist with coordinating care and resources for families experiencing crisis
  • Collaboration and communication with in building staff to facilitate transition
  • Provide regular updates and reports as require

Cross Agency Social Worker or Counselor Qualifications:

• Masters Degree in Social Work or School Counseling
• Valid license (LSW/LCSW) - Preference will be given to candidates with clinical licensure
• School Social Worker License or School Counselor License
• Preference will be given to candidates with at least 5 years of experience
• Preference given to candidates that are multi-lingual
• Strong interpersonal skills
• Working knowledge of county agencies including human services, school, health care, community support agencies, law enforcement, and juvenile justice
• Demonstrated positive connections with area families and students
• Solid understanding of mental health and applicable WI statutes
• Knowledge and experience in response to individuals in mental health crisis
• Knowledge of cross agency and state resources available to individuals that will assist with mitigation of factors contributing to crisis

District Description: The district includes Sauk City and Prairie du Sac, two "next door" villages known as Sauk Prairie. They are located on the beautiful Wisconsin River. Thirteen towns and the village of Merrimac are also in the district. The area features rolling hills, meadows, bluffs, forests, lakes and streams. The district offers small-town, rural living with easy access to the Madison urban area (25 miles) and to the Wisconsin Dells recreation area (28 miles). The district serves 2,600 students, and comprises six learning communities with a high school, middle school and four elementary schools.
