RN Case Manager Supervisor
- Full-Time, Monday-Friday 8am-5pm

The RN Case Manager Supervisor will provide day to day support to nurse case managers that collaborate with physicians, patients, and families promoting quality outcomes with complex medical needs.
Responsibilities:
- Provide professional clinical leadership to assigned care managers
- Supports new employee post orientation phase with direct in office supervision
- Assists in analyzing data collected from modeling tools, identifying coaching opportunities for assigned care managers
- Provides input and assist in staff evaluations
- Act as a resource for care managers, physicians, office staff regarding the role of care manager
- Promote a positive work environment
- Develop and maintain resource materials, provide content material for care ally meetings
- Perform additional duties and tasks as assigned
- Requires 75-80% travel from site to site of assigned care managers
Qualifications:
EDUCATION and / or CERTIFICATION/LICENSURE:
- Current Registered Nurse License in the State of Illinois
- Certification in Case Management, preferred
- Current CPR certification must be maintained (BLS).
EXPERIENCE:
- Case management experience preferred
- 2-4 years of experience working within the primary care or hospital setting as a Registered Nurse.
- Previous supervisor/manager experience preferred
The compensation for this role includes a base pay range of $72, $118,884.00, with the actual pay determined by factors such as skills, experience, education, certifications, geographic location, and internal equity. Additional compensation may be available through shift differentials, bonuses, and other incentives. Base pay is only a portion of the total rewards package.
